Circumstances:
Shortly after takeoff from Kazan Airport, while climbing, the crew encountered engine problems. The captain decided to maintain a low altitude and initiated a turn to the left when the engine failed completely 18 seconds later. While turning to the left at an angle of 20-27° and at a speed of 135 km/h, the airplane stalled, struck telephone wires and crashed in flames. The captain was killed while 10 other occupants were injured. The aircraft was destroyed.
Probable cause:
Engine failure at low height after takeoff over a populated area. The main cause of engine failure was a rupture of the cylinder n°8, which caused a leak and overheating. The presence of cracks in the right magneto slider BSM-9 also contributed to the engine failure. This caused the fuel-air mixture in the intake manifold to ignite.

Out of control, the aircraft crashed in flames in a field and was destroyed by impact forces and a post crash fire. Both pilots were killed.
Probable cause:
It was determined that the loss of control was caused by a stall due to an insufficient speed. The experience of the crew was too low and he failed to pass the last medical exam.

Circumstances:
While flying at an altitude of 4,200 metres from Luanda to Malabo, the crew received the last weather briefing for Malabo Airport and was cleared to descent for a landing on runway 05. ATC requested the crew to report once runway 05 in sight and the crew gave his ETA four minutes later at 0752LT. While approaching at an altitude of 750 metres and at a speed of 490 km/h, the airplane struck the south slope of Mt San Carlos located 48 km southwest of Malabo Airport. The wreckage was sighted by the pilot of the personal helicopter of the Equatorial Guinea President five days later, on June 6. SAR teams were dispatched and arrived on scene on June 18 only. The aircraft was totally destroyed by impact forces and a post crash fire and all 45 occupants have been killed.
Probable cause:
The exact cause of the accident could not be determined with certainty. ATC assistance was considered as good, all necessary information were transmitted to the crew and ATC was not the cause of the accident. The assumption that the aircraft radar was not operating at the time of the accident was not ruled out. Possible erroneous calculation on part of the crew may cause the aircraft to descent prematurely and to fly on the wrong path to runway 05. In consequence, the accident may be the result of a controlled flight into terrain.

The pilot and a passenger were killed while two other passengers were seriously injured. One of the survivor, a teenager, died a day later.

Circumstances:
On a flight from Vinnitsa to Moscow-Bykovo, while cruising at an altitude of 5,700 meters, the crew disengaged the autopilot system when the sliding cylinder of the MP-100 rudder trimming device deployed by 22.5 mm immediately, causing the rudder to move to the right, reaching the maximum position of 25° within one second. The aircraft went out of control, entered a spin and crashed at a speed of 350 km/h and a nose-down attitude of 50° in a field located 500 metres southeast of Viktorivka, 14.8 km southeast of Chernihiv Airport. The airplane disintegrated on impact and all 52 occupants were killed.
Probable cause:
Failure of the rudder trimming device during cruise flight. This may have been caused by a technical issue on the electrical system or an unintended pressure of the trim control switch by one of the crew members while the autopilot system was still ON.
