Circumstances:
The crew was performing a local training mission at Balashov Airport, in the region of Saratov. Following several touch-and-go manoeuvres, the crew initiated a new approach. On final, the airplane descended below the MDA and, at a speed of 240 km/h, rolled to the right to an angle of 24°, stalled and crashed in a field located 1,100 metres from the airfield, bursting into flames. Five occupants were injures while a pilot under supervision was killed. The airplane had the dual registration RF-36160 and 79 red.
Crew:
Cpt Y. Tereshin,
Maj S. Rodionov,
Ens Frolov,
P. Halaimov,
I. Makhmoudov,
M. Artemiev. †
Probable cause:
It was determined that during the approach, the instructor led the aircraft descending below MDA when he simulated an engine failure and positioned both power levers to idle. As a result of the failure of the propeller feathering system, the right engine stopped. After 14 seconds, the instructor mistakenly shut down the left engine that was running properly, causing the aircraft to lose speed, to stall and to crash.

Circumstances:
The airplane was engaged in a crop spraying mission, carrying a load of herbicides, one passenger and one pilot. In the early morning, while completing a seventh low pass, at a height of about 20-40 metres, the pilot initiated a left turn when the aircraft descended to the ground and crashed at a speed of 140 km/h in an open field, bursting into flames. The aircraft was destroyed by impact forces and a post crash fire and both occupants were killed.
Probable cause:
Fatal accident with Аn-2 RA-33462 agricultural aircraft occurred during aerial works on crop-dusting and was caused most probably by the piloting error consisting in unintentional descent when performing a turn for the next crop-dusting round which resulted in the aircraft controlled flight into terrain.
Most probably, the following factors contributed to the accident:
- The PIC not trained for crop-dusting flight without first officer,
- The PIC conducting the flight with a passenger on first-officer’s seat,
- The presence of alcohol in PIC's and the passenger's bodies,
- A possible transfer of the aircraft control from PIC to the passenger (former An-2 pilot) made by PIC,
- Violations of An-2 AFM and FAR-128 requirements on maintaining heights, bank angles during turns and flight speed while crop-dusting,
- Krasny Kut Civil Aviation Flight School management not controlling crop-dusting operations at the fields.

The crew decided not to land and initiated a go-around procedure. At a speed of 250 km/h, the aircraft climbed to a height of 200-250 meters when it stalled and crashed near the runway, bursting into flames. All three crew members were killed.
Probable cause:
The exact cause of the accident could not be determined. However, the assumption that the crew attempted a turn to avoid a flock of birds was not ruled out.

Circumstances:
While descending to Saratov-Tsentralny Airport, the crew contacted ATC and was cleared to continue the approach. In poor visibility due to bad weather (low clouds and rain falls), the crew failed to realize his altitude was insufficient when the airplane struck tree tops and crashed in a wooded area located 4,5 km west of the airport. Due to poor weather conditions, rescuers arrived on scene at 2200LT in the evening. The copilot and an illegal passenger were seriously injured while 14 other occupants were killed.
Probable cause:
It was determined that the crew continued the approach below weather minima and failed to see and avoid ground obstacles. The following contributing factors were reported:
- Wrong decision on part of ATC at Saratov Airport as the crew should be diverted to another airport and not cleared for approach,
- Information transmitted to crew regarding weather conditions and forecast were inaccurate,
- Poor flight operations conception,
- Poor flight preparation,
- Wrong decisions ont part of ATC and pilots,
- Poor weather conditions (limited visibility, low clouds, rain).

Circumstances:
A group of seven PZL-Mielec AN-2 must be transferred from Urgench, Uzbekistan, to Myashkovo, with intermediate stops in Uralsk and Penza, carrying mechanics and crew members. En route from Uralsk to Penza, while cruising at an altitude of 500 meters, both aircraft registered CCCP-28938 and CCCP-32112 entered a cloudy area with light rain. They collided then entered a dive and crashed in flames 350 meters further, 25 meters from each other. Both aircraft were totally destroyed and all 10 occupants were killed.
Probable cause:
Poor flight preparation and planning as all crew and the people in charge of the mission failed to analyze weather conditions en route. The collision occurred while both crew were flying under VFR mode in adverse weather conditions.
