Circumstances:
The pilot indicated that, prior to the accident flight, the wing flaps had failed, but he decided to proceed with the flight contrary to the Airplane Flight Manual guidance. While conducting a no-flap approach to the airport, he decided that his airspeed was too fast to land, and he initiated a go-around. During the go-around, the airplane continued to descend, and the fuselage struck the runway. The pilot was able to complete the go-around, and realized that he had not extended the landing gear. He lowered the landing gear, and landed the airplane uneventfully. He elected to remain overnight at the airport due to fatigue. The next day, he decided to test fly the airplane. During the takeoff roll, the airplane had a severe vibration, and he aborted the takeoff. During a subsequent inspection, an aviation mechanic discovered that the center wing carry-through cracked when the belly skid pad deflected up into a stringer during the gear-up landing.
Probable cause:
The pilot landed without lowering the landing gear. Contributing to the accident was the pilot's decision to fly the airplane with an inoperative wing flap system.

During the takeoff roll, at a speed of 290 km/h, the copilot started the rotation. The aircraft lifted off, reached a height of 7 metres then entered a left bank of 3,5°. Following a course of 665 metres, the aircraft entered a critical angle of attack, nosed up to 35° then landed back on the runway while the undercarriage have just been retracted. The aircraft slid on its belly and came to rest 270 metres further, about 120 metres to the left of the runway centerline. All 25 occupants were rescued, among them two were seriously injured. The aircraft was damaged beyond repair.
Probable cause:
It was determined that the captain decided to seat on the right front seat, letting the command to the copilot seating in the left front seat, which was against the published procedures because the copilot was not authorized and did not have sufficient experience. The copilot forgot to extend the flaps in an appropriate angle for the takeoff procedure, which caused the aircraft to adopt a critical angle of attack after liftoff and to stall. Successives errors and poor crew coordination were considered as contributing factors as well as the fact that the crew failed to follow the pre-takeoff checklist and failed to identify that the aircraft was not properly configured for the takeoff procedure.

The aircraft was completing a cargo flight from Magadan to Anadyr on behalf of the Federal Border Guard Aviation Command, carrying 11 passengers, seven crew members and a load consisting of helicopters spare parts. On final approach, the pilot informed ATC about the failure of the right engine and elected to make an emergency landing when the aircraft crash landed in a field located 4 km short of runway. On touchdown, the undercarriage were torn off and the aircraft slid for few dozen metres before coming to rest. All 18 occupants were rescued, among them four were injured. The aircraft was written off.
Probable cause:
Failure of the right engine on approach for unknown reasons.

The crew was performing a delivery flight from Ukraine to Honduras with several intermediate stops. On approach to Bukhta Provideniya, just after passing the outer marker, the aircraft sharply banked to the left. The left wing struck the ground and the aircraft crashed in a snow covered field some 2,100 metres short of runway threshold. All three occupants were injured while the aircraft was damaged beyond repair.
Probable cause:
It appears that the thrust reverser on the left engine mistakenly engaged on approach due to a mechanical failure.

The aircraft was performing a night cargo flight from Khatanga to Anadyr, carrying five passengers, six crew members and a load of vodka. The approach was completed in poor visibility due to heavy snow falls. Too high on the glide, the aircraft landed too far down the runway and was unable to stop within the remaining distance. It overran and came to rest in a ravine. All 11 occupants escaped with minor injuries and the aircraft was destroyed.
Probable cause:
Wrong approach configuration on part of the crew who should initiate a go-around as the aircraft was too high on the glide.

While in cruising altitude by night, the aircraft suffered violent oscillations and started to pitch up and down. It entered an uncontrolled descent and at an altitude of 2,000 metres, it partially disintegrated and eventually crashed at a speed of 670 km/h in an icy river. All 10 occupants were killed.
Probable cause:
It was determined that the crew disengaged and re-engaged several times the automatic pilot system for unknown reasons. This created tail and elevators' movements and the reaction of the crew was inappropriate. The aircraft suffered violent oscillations on both roll and pitch axis then entered an uncontrolled descent. The partial disintegration was the consequence of aerodynamic forces that exceeded the aircraft certification.

The crew left Anadyr Airport at 0919LT bound for Egvekinot but five minutes later, encountered poor weather conditions and decided to return for a safe landing. He left again Anadyr at 1213LT in better conditions. However, while approaching Egvekinot, the pilot encountered poor visibility due to low clouds and was unable to distinguish the mountainous terrain. While descending to a height of 235 meters, the aircraft deviated from the prescribed approach path and hit the slope of Mt Dumka, passed over the top and eventually crashed in flames on the opposite slope. SAR operations were conducted and the wreckage was localised a day later. Five passengers were seriously injured while 22 other occupants were killed.
Probable cause:
The captain took the decision to continue the approach below the minimum safe altitude of 300 meters despite the visibility was low due to poor weather conditions and that he did not have any visual contact with ground. The main approach radio beacon was out of order while the reserve one was running improperly, which was considered as a contributory factor.

The crew departed Mys Schmidta (Cape Schmidt) with 14,5 tons of fuel to Anadyr. Following an uneventful flight, a load of 10 tons of meat was loaded in Anadyr and the crew left Anadyr Airport at 0625LT bound for Magadan. One hour and twenty minutes into the flight, while cruising at an altitude of 7,000 meters, the radio operator informed ATC that the engine number two failed. The captain started an emergency descent and reach the altitude of 3,000 meters five minutes later when the engine number one failed as well. In such conditions, the crew elected to make an emergency landing on the icy Shelikov Gulf. The airplane belly landed on the ice and slid for several yards before coming to rest 2,700 meters off the coast. It went through the ice and sank. The wreckage and the crew was found six days later, on March 4 at 0830LT. The aircraft was lost and all ten occupants died from hypothermia (OAT -18° C and icy water).
Probable cause:
It was determined that a hot air line located near the engine number two broke in flight. The hot air caused the melting of electrical components and the development of a fire in the engine number two nacelle, followed by the rupture of a fuel hose. Unfortunately, the aircraft was not equipped with life jackets, signaling equipment or warm clothing.
