Shortly after takeoff from Kampala-Kajjansi Airport, the crew encountered technical issues and elected to return for an emergency landing. Upon touchdown, the single engine airplane went out of control, lost its left wing and engine before coming to rest upside down in a grassy area. Both pilots were rescued, the copilot was slightly injured and the captain was seriously injured. The crew departed Kajjansi Airfield on a positioning flight to Mweya to pick up passengers to Entebbe.

The airplane crash landed in an area located in the Hai Referendum district about 3 km northwest of the airport. All five occupants escaped with minor injuries while the airplane was severely damaged.

On final approach to Bukoba Airport, the crew encountered poor visibility due to heavy rain falls. In circumstances that need to be clarified, the airplane descended too low, impacted the water surface and crashed into Lake Victoria, about 100 metres short of runway 31. Fishermen with several boats were quickly on the scene and were able to evacuate 24 people. Unfortunately, 19 others including both pilots were killed. The aircraft sank by a depth about 3-4 metres.

The airplane departed Kasese Airport at 1500LT on a cargo flight to Goma, carrying two pilots, one passenger and a load of various goods for a total weight of one ton. En route, weather conditions deteriorated with low clouds and heavy rain falls. As the airplane failed to arrive at destination, SAR operations were initiated and the wreckage was found two days later in a dense wooded area located in South Kivu, about 139 km southwest of Goma. The airplane was destroyed by a post impact fire and all three occupants were killed.

Investigations were unable to determine the exact cause of the accident. Nevertheless, it may be the consequence of an insufficient consideration of weather conditions during flight preparation or during the flight itself, to a system or component failure, or to human error (error of judgment and decision-making).
