Circumstances:
The aircraft was completing a cargo flight from Petropavlovsk-Kamchatsky to Novosibirsk with intermediate stops in Okha and Chita, carrying a load consisting of three Toyota vehicles and boxes of frozen fish. While completing a turn to join the approach path, one engine failed. The crew continued the approach when, on short final, the second engine failed as well. The aircraft lost height and crashed 1,657 metres short of runway. A passenger was seriously injured while 13 other occupants were killed. Few hours later, the only survivor died from his injuries.
Probable cause:
Double engine failure on approach caused by a fuel exhaustion. At the time of the accident, the total weight of the aircraft was 5 tons above the max landing weight.

Circumstances:
Shortly after takeoff from Ust-Nem Airport, while climbing to a height of about 30 metres, the left engine failed and its propeller autofeathered. After the crew retracted the flaps, the aircraft rolled to the left, adopted a high angle of attack then struck trees and crashed in a wooded area located 500 metres from the runway, bursting into flames. A passenger was injured while 15 other occupants were killed.
Probable cause:
It is believed that the left engine failed during initial climb after wet snow entered the air intake. The takeoff procedure was completed from a runway covered with slush and wet snow. The following contributing factors were reported:
- The crew retracted the flaps prematurely,
- Lack of crew training on this type of aircraft,
- The distance with ground was insufficient to expect recovery.

Circumstances:
The aircraft was completing a charter flight from Vladivostok to Minsk, carrying 62 Lithuanian tourists and five crew members. During the takeoff roll, at a speed of 320 km/h, the captain started the rotation but the aircraft failed to respond. The crew initiated an emergency braking procedure 1,000 metres from the runway end (the runway is 3,500 metres long). Unable to stop within the remaining distance, the aircraft overran at a speed of 200 km/h, lost its nose gear and came to rest 283 metres further. All 67 occupants evacuated safely while the aircraft was damaged beyond repair.
Probable cause:
It was determined that the aircraft was unable to rotate and takeoff because its total weight was about 6,8 tons above MTOW and the CofG was too far forward out of the enveloppe. Investigations revealed that the flight was not prepared according to published procedures and that the weight and balance sheet provided to the crew did not reflect the truth as a total of 16,800 kg of cargo and luggages were boarded instead of the 10,000 mentioned on the documentation.

The aircraft departed Mineralnye Vody on a schedule service to Ivanovo with an intermediate stop in Donetsk. Following an uneventful flight, the crew initiated the descent to Ivanovo-Yuzhny Airport. The visibility was limited due to the night and poor weather conditions. On final approach, the aircraft was not properly aligned on the glide and the copilot and the navigator attempted to inform the captain about the deviation but without success. The captain continued the approach in a wrong configuration when, at a speed of 370 km/h and a rate of descent of 6 metres per second, the left wing struck a residential building. The aircraft entered a nose-down attitude and crashed 512 metres further, some 2,450 metres short of runway 29, bursting into flames. The aircraft was totally destroyed and all 84 occupants were killed.
Probable cause:
The accident was the consequence of a wrong approach configuration on part of the crew. The captain ignored several alerts from the copilot and the navigator and failed to initiate a go-around procedure despite the GPWS alarm sounded for about 10 seconds. The following contributing factors were reported:
- Loss of altitude control in adverse weather conditions following a premature descent,
- The crew failed to follow the published approach procedures, which resulted in the aircraft approaching the airport at an insufficient altitude and at an excessive speed,
- The 4th turn was completed too late at a distance less than calculated, which caused the aircraft to deviate from the approach path with a separation (deviation) of 1,900 metres to the right of the approach path at a distance of 8,600 metres from the runway threshold at an altitude of 500 metres,
- The captain decided to continue the approach while a landing was obviously impossible,
- A total interruption of interactions within the crew during the approach procedure, which brang the captain in a delicate situation without any assistance from the other members of the crew and causing a regular loss of the altitude control,
- The crew let the aircraft passing the admissible vertical speed during the final approach,
- Failure of the crew to initiate a go-around procedure while the Ground Proximity Warning System alarm sounded for about 10 seconds,
- Excessive corrective maneuvers on part of the captain which caused the aircraft to enter a roll angle greater than the permissible limits,
- The violation by the crew of the approach pattern was facilitated by the lack of assistance from ATC at Ivanovo Airport.

The pilot, sole on board, was completing a crop-spraying flight in Leninsky, district of Timashevsk. En route, the engine lost power then failed. The pilot reduced his altitude in an attempt to make an emergency landing when the right wing struck trees, causing the aircraft to crash. The pilot was injured and the aircraft was destroyed.
Probable cause:
Engine failure after particles of rubber sealing rings went into the cylinder head, causing the failure of cylinder n°4 and 6.

Circumstances:
The four engine aircraft departed Magadan Airport on a cargo flight to Irkutsk, carrying one loadmaster, six crew members and a load of 11 tons of fish. For financial reasons, the crew refused to make a refueling stop at Lensk Airport and decided to continue to Bratsk. Unfortunately, weather conditions worsened at Bratsk and a landing was impossible. In such conditions, the captain decided to continue to Irkutsk and later shut down both engines n°1 and 4 to save fuel. While descending to Irkutsk, both engines n°2 and 3 failed due to fuel exhaustion. The crew reduced his altitude and attempted an emergency landing in an open field located 48 km from Irkutsk Airport. The aircraft belly landed, slid for few dozen metres and eventually came to rest, broken in two. All seven occupants escaped uninjured.
Probable cause:
Engine failure due to fuel exhaustion.
