After takeoff from Ivanovo-Severny AFB, while climbing, the engine n°4 caught fire. The crew decided to return to the base and initiated a circuit in a slight right bank attitude. Few dozen seconds after departure, the engine n°4 detached. The airplane later entered an uncontrolled descent and crashed in a wooded area, bursting into flames. All 16 occupants were killed. The crew was en route to the airbase of Plessetsk, Arkhangelsk oblast.

Just after liftoff, at a height of about 3 meters, left engine thrust reverser deployed. The takeoff procedure was aborted but the aircraft veered off runway and came to rest in a field with the cockpit partially separated. All five occupants were injured, both pilots seriously. The aircraft was damaged beyond repair. Configured in a VIP version, it was used by the FSB's Director Nikolai Patrushev and flying back to Moscow at the time of the accident.

Probable cause:

The thrust reverser deployed accidentally on the left engine after rotation due to inappropriate maintenance.

The crew lined up on runway and was cleared for takeoff. Power was added on all four engine and the crew started the takeoff roll when the engine n°3 detached from its pylon, fall on the ground and rolled for about 150 metres before coming to rest. The crew stopped the airplane and evacuated safely. The aircraft was damaged beyond repair.

Probable cause:

The engine n°3 detached during the takeoff roll for unknown reasons.

Circumstances:

The aircraft departed Mineralnye Vody on a schedule service to Ivanovo with an intermediate stop in Donetsk. Following an uneventful flight, the crew initiated the descent to Ivanovo-Yuzhny Airport. The visibility was limited due to the night and poor weather conditions. On final approach, the aircraft was not properly aligned on the glide and the copilot and the navigator attempted to inform the captain about the deviation but without success. The captain continued the approach in a wrong configuration when, at a speed of 370 km/h and a rate of descent of 6 metres per second, the left wing struck a residential building. The aircraft entered a nose-down attitude and crashed 512 metres further, some 2,450 metres short of runway 29, bursting into flames. The aircraft was totally destroyed and all 84 occupants were killed.

Probable cause:

The accident was the consequence of a wrong approach configuration on part of the crew. The captain ignored several alerts from the copilot and the navigator and failed to initiate a go-around procedure despite the GPWS alarm sounded for about 10 seconds. The following contributing factors were reported:
- Loss of altitude control in adverse weather conditions following a premature descent,
- The crew failed to follow the published approach procedures, which resulted in the aircraft approaching the airport at an insufficient altitude and at an excessive speed,
- The 4th turn was completed too late at a distance less than calculated, which caused the aircraft to deviate from the approach path with a separation (deviation) of 1,900 metres to the right of the approach path at a distance of 8,600 metres from the runway threshold at an altitude of 500 metres,
- The captain decided to continue the approach while a landing was obviously impossible,
- A total interruption of interactions within the crew during the approach procedure, which brang the captain in a delicate situation without any assistance from the other members of the crew and causing a regular loss of the altitude control,
- The crew let the aircraft passing the admissible vertical speed during the final approach,
- Failure of the crew to initiate a go-around procedure while the Ground Proximity Warning System alarm sounded for about 10 seconds,
- Excessive corrective maneuvers on part of the captain which caused the aircraft to enter a roll angle greater than the permissible limits,
- The violation by the crew of the approach pattern was facilitated by the lack of assistance from ATC at Ivanovo Airport.

Shortly after takeoff from Ivanovo Airport, while climbing in poor weather conditions, the pilot-in-command lost control of the airplane that stalled and crashed in a field. The crew fate remains unknown while the aircraft was destroyed.

Probable cause:

It was determined that the loss of control occurred during initial climb in poor visibility due to bad weather when the crew failed to prepare the flight according to published procedures and forgot to trigger the artificial horizon.

Circumstances:

During the day, the crew completed crop spraying missions for the sovkhoz of 'Garsky' located in the region of Sokolsky. At 1713LT, the airplane departed Sokolsky on a ferry flight to Ivanovo with three crew members and one unauthorized passenger on board. While cruising at a relative low altitude, the single engine airplane went out of control and crashed in flames in a wooded area located 2 km northwest of Degtyarnovo. The burnt wreckage was found in the evening and all four occupants have been killed.

Probable cause:

It was determined that all four occupants (both pilots, the mechanic and the passenger) were intoxicated at the time of the accident. The crew decided to embark a passenger at Sokolsky without prior permission and all of them consumed alcohol during all day and prior to departure. A lack of discipline, a high alcohol level in blood and a total distraction caused the pilot to lose control of the aircraft. The alcohol level was 5,4‰ by the captain, 2‰ by the copilot, 4,4‰ by the mechanic and 2,5‰ by the passenger.

Circumstances:

The crew was performing a cargo flight from Moscow-Chkalovski to Belomorsk with an intermediate stop in Vologda. About 35 minutes after takeoff, while cruising at an altitude of 200-250 meters, the right engine lost power and stopped. Shortly later, the left engine also failed. The Captain attempted to make an emergency landing when the aircraft hit tree tops and crashed in flames in a wooded area. Four crew members and one passenger were killed while three other occupants were injured. The aircraft was destroyed.

Probable cause:

The flight mechanic forgot to switch the tanks in time sot the fuel pressure dropped, causing the right engine to fail, soon followed by the left one.
