Circumstances:
While cruising parallel to the Seward - Anchorage highway at an altitude of 1,000 feet, the twin engine airplane collided with a Kenai Civil Air Patrol Beechcraft A-45 Mentor registered N1406Z. Engaged in a SAR mission after a Cessna 170 was missing, the Mentor was carrying two pilots. Following the collision, both aircraft dove into the ground and crashed. All four occupants were killed.
Probable cause:
Collision with other aircraft in normal cruise. The following contributing factors were reported:
- Failed to see and avoid other aircraft,
- Pilot of other aircraft,
- Not under radar contact,
- No control.

Circumstances:
En route by night from Austin to Houston, the pilot encountered poor weather conditions and decided to continue. Later, the twin engine airplane struck tree tops and crashed near Giddings. The aircraft was destroyed by impact forces and both occupants were killed.
Probable cause:
Collision with trees in normal cruise due to improper in-flight decisions on part of the pilot. The following contributing factors were reported:
- Continued VFR flight into adverse weather conditions,
- High obstructions,
- Low ceiling,
- Fog,
- Thunderstorm activity,
- Flew into known poor weather conditions.

Circumstances:
The aircraft was engaged in a local training flight, carrying one pilot under supervision, one instructor and one pilot from the Civil Aviation Authority. Shortly after takeoff from runway 07, while climbing, the twin engine airplane went out of control and crashed in flames in a wooded area located 2 km northeast of the airfield, near the farm called La Fuente. The aircraft was totally destroyed by impact forces and all three occupants have been killed.

Circumstances:
On approach to Elko, the pilot encountered poor weather conditions with low ceiling, snow falls, turbulences associated with clouds and thunderstorm. He descended below the MDA until the twin engine airplane struck a hill located few miles from the airport. The wreckage was found three days later on December 2. Both occupants have been killed.
Probable cause:
Controlled flight into terrain following improper IFR operation. The following contributing factors were reported:
- Failed to follow approved procedures,
- Low ceiling,
- Snow,
- Turbulences associated with clouds and thunderstorm
- High obstructions,
- Blowing snow,
- The pilot did not use available instrument approach chart for reference.

Circumstances:
The twin engine airplane was completing a charter flight from Göteborg to Gävle, carrying one pilot and seven players of the Västra Frölundas Hockey Team. On final approach to Gävle Airport in good weather conditions, both engines failed simultaneously. The pilot elected to make an emergency landing when the airplane struck tree tops and crashed in a wooded area located few km from the airfield. All eight occupants were injured and rescued about two hours later while the aircraft was destroyed.

On approach to Dodge City Airport in marginal weather conditions, the the crew decided to initiate a go-around when the airplane struck power cables, stalled and crashed in an open field. All three occupants were injured and the aircraft was destroyed.
Probable cause:
The crew failed to follow the approved procedures while on an IFR approach. The following contributing factors were reported:
- Improper in-flight decisions,
- Ice on windshield,
- Airframe ice,
- Icing conditions including sleet and freezing rain,
- Disregard of good operating practice,
- Fog,
- Visibility reference established windshield deicer turned off,
- Windshield iced over,
- Unable to continue the flight due to ice accumulation.
