Circumstances:
The crew departed Krechevitsky AFB on a local training flight. After several materials have been dropped at low height, the crew elected to gain altitude when he encountered technical problems with the electrical system. The aircraft nosed up to an angle of 26° then climbed from 940 meters to 1,300 metres in few seconds. Suddenly, it adopted a nose down attitude of 55°, entered a dive and crashed at a speed of 570 km/h. The wreckage was found between the villages of Rogavka and Kerest, about 32 km northwest of the airbase. The aircraft disintegrated on impact and all seven crew members were killed.
Probable cause:
It is believed that the electrical system failed in flight, maybe after the aircraft has been struck by lightning. At the time of the accident, visibility was limited due to poor weather conditions. After the electrical system failed, the pilot-in-command suffered a spatial disorientation and lost control of the airplane.

Circumstances:
The crew was engaged in a crop spraying mission for the kolkhoz of 'Rodina' located in Dovorets, in the region of Novgorod. After completing 15 rotations, the crew departed for a new spraying flight when, at a height of 20 meters, the single engine airplane struck power cables located 1,600 meters from the village. It crashed 636 metres further and caught fire. Both pilots were seriously injured and evacuated to a local hospital. The copilot died few hours later and the captain a day later.
Probable cause:
Due to lack of evidences, it was not possible to determine the exact cause of the accident.

The AN-12 departed Šiauliai Airport with two other AN-12 to take part to a training program, consisting of SU-11 fighter interception. While cruising at an altitude of 6,350 metres, the AN-12 collided under unknown circumstances with a SU-11. Both aircraft went out of control, entered a dive and crashed near Kresttsy. All eight crew members on board the AN-12 were killed as well as the SU-11's pilot.

Circumstances:
The crew encountered poor weather conditions while approaching Novgorod Airport with fog and a visibility below minimums. On final, the crew descended below MDA when the airplane struck the roof of a printing house and crashed in flames on a small residential building (three floors) located three km short of runway. The aircraft and the building were destroyed. All six occupants as well as five people in the building were killed.
Probable cause:
Failure of the crew to adhere to the published procedure while descending below the MDA in below minima visibility. Due to fog, the crew failed to locate the runway but continued the approach instead of initiating a go-around procedure. Weather information transmitted to the crew did not reflect the truth, which was considered as a contributing factor as well as the lack of ATC assistance.

Circumstances:
During a crop spraying flight from Zaupory (Batetski district of the Novgorod region) for the sovkhoz (state farm) of "Peredolski", the engine lost power while the aircraft was flying at low height. The airplane lost height then collided with the roof of a horse stable before coming to rest on the banks of the river Luga located near Rechka. Both pilots were killed.
Probable cause:
The engine failure was caused by soot deposits in the inlet valves.

Enroute from Vnukovo to Leningrad, while cruising at an altitude of 900 meters in clouds, the right engine failed. Unfortunately, the crew was unable to feather the propeller and thus, was unable to maintain a safe altitude. The captain decided to divert to the Vypolzovo Airbase but radio contact with the base was impossible for unknown reason. Eventually, the pilot attempted an emergency landing in a potatoes field located in Ozerevo when the airplane crashed. Five crew members and two passengers were killed while 12 other occupants were injured. The aircraft was destroyed.
Probable cause:
Failure of the right engine in flight caused by an oil leakage due to the failure of a connection hose.

About an hour after his departure from Khvoynaya Airport, en route by night to Leningrad, the pilot lost his orientation and decided to return to Khvoynaya. For unknown reason, he started an approach to the Yakhnovo Airfield located 8 km southwest of Khvoynaya. On final approach, while at a height of 30 meters, the aircraft went out of control, dove into the ground and crashed in a swampy area located 1,5 km from the runway threshold. The aircraft was destroyed and all 17 occupants were killed.
Probable cause:
It was determined that the loss of control was caused by the simultaneous failure of both engines on final approach. This was due to an error on part of the flight mechanic who failed to switch the tanks in due time as there was still sufficient fuel in other tanks (about 150 to 200 kg). It was reported that the flight mechanic was tired because he made other flights the same day and failed to have appropriate rest time. Also, he was appointed to this mission despite his experience with night flights was insufficient.

In the late hours of August 28, 1943, on a flight from Khvoynaya to the area of Lake Onega when was called back due to poor weather (although the weather at Khvoynaya was even worse). The crew elected to land during a thunderstorm with heavy rain and strong cross winds and had to go around. The pilot in command was blinded by a lightning and lost spatial orientation so the aircraft banked to the left and crashed. Two crew members were killed while four other were injured.

While returning to base following a night mission to the west front, the crew encountered poor weather conditions with snow falls, strong winds and icing. In a visibility of 30-50 metres, the airplane got iced, went out of control and crashed near the village of Pola, bursting into flames. The pilot was killed and the navigator was injured.
