Shortly after takeoff from Tel Aviv-Sde Dov Airport, while climbing, the pilot encountered technical problems and declared an emergency. He elected to return to the airport but eventually ditched the airplane few dozen metres offshore. Both occupants were rescued and the aircraft was damaged beyond repair.

Circumstances:
Following an uneventful flight from Herzliya, the pilot started the approach to Eilat Airport in excellent weather conditions. During the last segment, at a height of about 10 feet, the pilot initiated a go-around procedure following a misunderstanding about an ATC transmission that was given to another crew. The pilot mistakenly closed to fuel valve coupled to the right engine while trying to switch fuel tanks, causing the right engine to stop. Due to an asymmetric thrust, the pilot lost control of the airplane that veered to the right and crashed in a drainage ditch, coming to rest upside down. While both passengers were seriously injured, the pilot was killed.

On approach to Tel Aviv-Ben Gurion Intl Airport, the right main gear remained stuck in its wheel well. The crew informed ATC about the situation and continued the approach. After touchdown, the aircraft slid for few dozen metres then veered off runway to the right and came to rest. All 42 occupants escaped uninjured while the aircraft was damaged beyond repair.

After takeoff from Haifa Airport, while climbing to a height of about 300 feet, the engine lost power then failed. The aircraft lost height and crashed on the top of a hill near the airport. The pilot and three skydivers were killed while six other occupants were injured.
Probable cause:
Engine failure for unknown reasons.

After landing on runway 33 at Rosh Pina Airport, the captain thought the thrust reverser systems did not work properly so he instructed the copilot to activate them again. The aircraft failed to decelerate correctly so the captain decided to veer off runway when the aircraft struck a concrete block, lost its undercarriage and came to rest. All eight occupants escaped uninjured and the aircraft was damaged beyond repair.
Probable cause:
Runway 33 at Rosh Pina Airport is 1,100 metres long with a displaced threshold, giving a landing distance available of 975 metres. It was determined that the crew never land in the past at this airport and due to a poor flight preparation and planning, failed to identify that the runway length was insufficient for such type of aircraft in such flight configuration. No malfunction was found on the thrust reversers.

During the takeoff roll on runway 34, at a speed of 65 mph, the pilot pulled up on the control column to rotate. The nose gear lifted off briefly before touching down again. In such conditions, the pilot decided to abort the takeoff and initiated an emergency braking manoeuvre. Unable to stop within the remaining distance (the aircraft already rolled for 2/3 of the runway), the aircraft overran and came to rest in a rocky and bushy area. All 11 occupants evacuated safely while the aircraft was damaged beyond repair.
Probable cause:
It was determined that the pilot failed to prepare the flight according to procedures. No weight & balance sheet was calculated prior to departure. At the time of the accident, the total weight of the aircraft was above MTOW and its CofG was too far forward the permissible limit.

While parked at Tel Aviv-Sde Dov Airport, the aircraft was stolen by Captain Haggai Morri (33) who attempted to defect. En route over Syria, he was driven off from Syrian airspace by Syrian fighters and eventually decided to commit suicide. The aircraft crashed on the west part of the Golan Heights and the pilot was killed.
Probable cause:
Pilot's suicide.
