Circumstances:
The twin engine airplane departed Canela Airport runway 24 at 0912LT on a private flight to Jundiaí, carrying nine passengers and one pilot. Weather conditions were marginal with limited visibility due to rain falls and fog. One minute later, the airplane collided with the chimney of a private house, lost height and crashed on an inn located in the center of the city of Gramado, about 3 km west of Canela Airport. The airplane was totally destroyed as well as several buildings. All 10 occupants were killed and 17 people on the ground were injured, two seriously. The pilot and owner of the airplane, Luiz Claudio Salgueiro Galeazzi, was travelling with his wife, three daughters, sister, brother in law, mother in law and two nephews.

Circumstances:
The flight crew reported that, the instrument approach was flown on autopilot to about 700 ft above ground level until the runway was visually in sight. They were 300 ft off the runway centerline, and 1 nautical mile from the runway threshold. The visual glideslope indicator was inoperative, and the runway markings were obscured due to dry light snow. The airplane subsequently landed hard on the unusable portion of the runway, about 800 ft short of the landing threshold, and the left mail landing gear tire blew, causing the propeller to strike the runway. The airplane veered off the runway substantially damaging the left wing. The pilots reported that there were no preaccident mechanical failures or malfunctions with the airplane that would have precluded normal operation.
Probable cause:
The pilot’s improper landing flare, which resulted in a hard landing. Contributing to the accident was the out of service visual glideslope indicator and snow obscuring the runway markings.

The twin engine aircraft departed Manaus-Aeroclub de Flores Airport on a flight to Barra do Garças, carrying one passenger and one pilot. After takeoff from Manaus, the pilot changed his mind and decided to fly to Sorocaba. On final approach to Sorocaba-Bertram Luiz Leupolz Airport in good weather conditions, the aircraft impacted trees and crashed in a wooded area located about one km short of runway 18. The aircraft was destroyed and both occupants were killed. There was no fire.

- Flight indiscipline – a contributor
The pilot failed to comply with the minimum fuel requirements laid down in the regulations, providing conditions for both engines to stop operating in flight, due to lack of fuel.
- Piloting judgment – a contributor
It was found in this flight an inadequate evaluation for certain parameters related to aircraft operation, particularly with regard to the influence of the chosen flight level on fuel consumption. This misjudgment led to the decision to proceed with the flight to the Aerodrome where it was intended to land, to the detriment of the more conservative option of finding a suitable place for an intermediate landing and a refueling, which led to the depletion of usable fuel in flight.
- Flight planning – a contributor
Inadequate flight preparation work, especially with regard to fuel calculation and cruise level selection, has degraded the safety level and also contributed to the actual accident.
- Decision-making process – undetermined
Difficulties in perceiving, analyzing, choosing alternatives, and acting appropriately due to inadequate judgment, may have resulted in poor assessment of flight parameters (available fuel, distance to destination, verified consumption, etc.), which may have favored the occurrence of lack of fuel failure.

Shortly after takeoff from Tel Aviv-Sde Dov Airport, while climbing, the pilot encountered technical problems and declared an emergency. He elected to return to the airport but eventually ditched the airplane few dozen metres offshore. Both occupants were rescued and the aircraft was damaged beyond repair.
