Circumstances:
The twin engine airplane departed Sorocaba-Estadual Bertram Luiz Leupolz Airport at 2030LT on a flight to Palmas, Tocantins, carrying four passengers and one pilot. Few minutes after takeoff, the "FIRE" warning light on the alarm panel, related to the right engine of the aircraft, illuminated. The pilot shut down the right engine, declared an emergency and was cleared to divert to Campinas-Viracopos Airport. On approach, the airplane descended too low and the pilot made an emergency landing in a prairie located about 7 km short of runway 15 threshold. The wreckage was found less than a km from the Jardim Bassoli condominium and all five occupants, slightly injured, were evacuated. The aircraft was damaged beyond repair.

Contributing Factors:
- Control Application - contributed.
The difficulty in coordinating the aircraft due to its single-engine flight condition indicated that improper control application contributed to the outcome of this event. Additionally, the early configuration of the aircraft, with the landing gear lowered and flaps at 100%, led it to descend on a glide path below the recommended one, affecting the achievement of the necessary parameters for landing.
- Training - contributed.
The training process previously received by the PIC (Pilot in Command) was insufficient, as it did not provide them with the full knowledge and technical conditions required for performing the duties of a pilot in command of the EMB-121 A1, either due to a lack of training or the low quality of the training received.
- Pilot Judgment - contributed.
The inadequate training led to improper evaluations of the parameters related to the aircraft's operation, contributing to the event. This included a poor assessment of the situation and lowering the landing gear and setting flaps to 100% without ensuring that the landing was secured.

Circumstances:
The twin engine aircraft departed Manaus-Aeroclub de Flores Airport on a flight to Barra do Garças, carrying one passenger and one pilot. After takeoff from Manaus, the pilot changed his mind and decided to fly to Sorocaba. On final approach to Sorocaba-Bertram Luiz Leupolz Airport in good weather conditions, the aircraft impacted trees and crashed in a wooded area located about one km short of runway 18. The aircraft was destroyed and both occupants were killed. There was no fire.

The pilot failed to comply with the minimum fuel requirements laid down in the regulations, providing conditions for both engines to stop operating in flight, due to lack of fuel.
- Piloting judgment – a contributor
It was found in this flight an inadequate evaluation for certain parameters related to aircraft operation, particularly with regard to the influence of the chosen flight level on fuel consumption. This misjudgment led to the decision to proceed with the flight to the Aerodrome where it was intended to land, to the detriment of the more conservative option of finding a suitable place for an intermediate landing and a refueling, which led to the depletion of usable fuel in flight.
- Flight planning – a contributor
Inadequate flight preparation work, especially with regard to fuel calculation and cruise level selection, has degraded the safety level and also contributed to the actual accident.
- Decision-making process – undetermined
Difficulties in perceiving, analyzing, choosing alternatives, and acting appropriately due to inadequate judgment, may have resulted in poor assessment of flight parameters (available fuel, distance to destination, verified consumption, etc.), which may have favored the occurrence of lack of fuel failure.

Shortly after take off from Sorocaba Airport, while climbing, the pilot encountered technical problems and elected to return. While trying to land in a wasteland, the single engine aircraft crashed in a street and was destroyed by impact forces and a post impact fire. Both occupants were killed as a house was also destroyed.

Circumstances:
The twin engine aircraft departed Sorocaba on a cargo flight to Cascavel, carrying two pilots and a load consisting of pharmaceutical materials. On final approach in low visibility due to bad weather conditions and night, the aircraft descended below the glide and impacted the ground 700 metres short of runway 33. On impact, it lost its undercarriage then slid for 150 metres before coming to rest. Both pilots escaped uninjured while the aircraft was damaged beyond repair. Visibility at the time of the accident was 1,200 metres with mist, local patches of fog and ceiling at 100 feet. Cascavel Airport was equipped with an NDB only.
Probable cause:
Controlled flight into terrain after the crew descended too low in IMC conditions. The following contributing factors were identified:
- Visibility was below minimums,
- The crew continued the descent until the aircraft impacted ground and failed to initiate a go-around procedure,
- A probable crew fatigue,
- It is possible that the crew suffered optical illusions,
- Overconfidence on part of the captain,
- The captain did not request any assistance from the copilot during the approach procedure,
- Inexperienced, the copilot did not interfere despite dangerous flight conditions,
- Poor organizational culture,
- Deficiencies in crew training,
- Lack of crew discipline,
- Poor flight planning,
- Lack of supervision on part of the operator,
- The copilot was inexperienced on this type of aircraft.

Circumstances:
The aircraft departed Lins Airport on a ferry flight to Sorocaba with two pilots on one passenger (the owner) on board. The aircraft was transferred to Sorocaba Airport for maintenance purposes. While descending, the crew was informed that runway 36 was in use and that three small aircraft were completing local training in the circuit. In good weather conditions, the captain decided to complete a straight-in approach to runway 18. After touchdown, the aircraft was unable to stop within the remaining distance. It overran, crossed a road and came to rest into a ravine. The passenger escaped uninjured, the copilot was seriously injured and the captain was killed. The aircraft was destroyed.
Probable cause:
Wrong approach configuration on part of the crew who completed an unstabilized approach and landed too far down the runway (about a half way down) at an excessive speed. In such conditions, the aircraft could not be stopped within the remaining distance. The following contributing factors were identified:
- The crew did not make any approach briefing,
- The crew failed to follow the approach checklist,
- The aircraft had deficiencies in maintenance, particularly with regard to the brakes systems,
- The techlogs were out of date,
- Maintenance was periodic but insufficient,
- Although the runway 36 was in use, the captain preferred to land on runway 18,
- The aircraft was unstable on short final and landed too far down the runway, reducing the landing distance available,
- The aircraft' speed upon landing was excessive, preventing the reverse thrust systems to be activated,
- The captain took over control and activated the reverse thrust system on the right engine only,
- Poor crew coordination,
- The crew was operating in a conflict environment after touchdown,
- Poor judgment of the situation,
- Poor flight planning,
- Lack of crew discipline.

Shortly after takeoff from Belo Horizonte-Pampulha Airport, while in initial climb, the left engine failed. The pilot lost control of the aircraft that rolled to the left, lost height and crashed onto four houses located in the district of São Bernardo near the airport. The aircraft and four houses were destroyed. All three occupants were killed and two other people on the ground were injured.
Probable cause:
Failure of the left engine that was not lubricated according to published procedures. Inappropriate engine maintenance on part of the operator. Also, it was reported that the pilot's experience on this type of aircraft was insufficient.
