Shortly after takeoff from Gander Airport, while in initial climb, the twin engine airplane encountered difficulties to gain height. It struck trees, stalled and crashed in flames in a wooded area. The aircraft was destroyed by a post crash fire and both pilots, en route to Europe, were killed.

Circumstances:
While descending to Aspen Airport, the pilot encountered marginal weather conditions. Passing below the clouds, he became lost. While completing a last turn to the left, the airplane struck trees and crashed on a snow covered mountain located near Glenwood Springs, northwest of Aspen. The wreckage was found two days later in an isolated area. Two passengers were seriously injured while five other occupants were killed.
Probable cause:
Collision with trees and terrain after the pilot became lost/disoriented. The following factors were reported:
- Improper in-flight decisions or planning,
- Misjudged speed and clearance,
- Low ceiling,
- Downdrafts, updrafts,
- High obstructions,
- Snow covered terrain.

Probable cause:
Uncontrolled descent and ground collision following a battery failure (electric system) in flight. The following factors were reported:
- Overheated,

- Instruments misread or failed to read,
- Thermal runaway and fire in n°2 nickel-cadmium battery in right wheel well,
- Pilot lost control for undetermined reason.

Circumstances:
Shortly after takeoff from a grassy runway located in Northfield, the twin engine airplane struck tree tops, stalled and crashed. All four occupants were injured, the pilot seriously. The aircraft was destroyed, it was en route to Cleveland-Hopkins Airport. It was determined that the pilot started the takeoff run from a muddy runway with a too short takeoff distance.
Probable cause:
Inadequate preflight preparation on part of the pilot who failed to obtain/maintain flying speed during initial climb. The following factors were reported:
- Airport conditions, soft runway,
- Failed to use all available runway,
- High obstructions,
- All runways were muddy,
- Began takeoff roll 690 feet short of departure end due to mud ground,
- Presence of 92 feet trees about 60 feet from the runway end.
