Circumstances:
The aircraft departed Kanoya AFB at 1315LT on a calibration flight with six people on board. After he complete a counter clockwise arc, the crew started the descent to Kanoya AFB Runway 08R. The visibility was poor due to low clouds. On approach, at an altitude of 3,000 feet, the aircraft entered clouds when the GPWS alarm sounded. Two second later, the crew deactivated the alarm and continued the approach. Ten seconds later, the aircraft impacted trees and crashed on the slope of Mt Takakuma (1,182 metres high) located 10 km north of the airbase. The wreckage was found a day later and all six crew members were killed.
Probable cause:
Controlled flight into terrain after the crew continued the approach in poor visibility without visual contact with the environment. Misidentification of the environment on part of the crew was a contributing factor, as well as the fact that the crew deactivated the GPWS alarm and failed to initiate corrective maneuver.

Circumstances:
The single engine aircraft departed Kumamoto Airport runway 07 at 1711LT on a private flight to Kitakyūshū, with two persons on board: a PIC in the left seat and a passenger in the right seat. At 17:12:11, the radar of the Kumamoto aerodrome station C captured the aircraft. The pilot made a position report at 6 nm north of the airport at 2,300 feet. While climbing to the altitude of 6,500 feet, the pilot was instructed to change the frequency. About three minutes after takeoff, the aircraft collided with trees and crashed in a wooded area located on the southeast slope of Mt Yago, about 14 km northeast of Kumamoto Airport. The wreckage was found in the afternoon of the following day at an altitude of 850 metres. The aircraft was destroyed and both occupants were killed, Mr. & Mrs. Hiroshi and Hiromi Kanda.
Probable cause:
It is highly probable that the aircraft collided with the mountain slope during its in-cloud post-takeoff climb with low climb rate on its VFR flight to Kitakyushu Airport from Kumamoto Airport, resulting in the aircraft destruction and fatal injuries of two persons on board–the PIC and the passenger. It is somewhat likely that the contributing factor to in-cloud flight toward mountain slope with low climb rate is the PIC’s lack of familiarization with terrain features near Kumamoto Airport; however, the JTSB was unable to clarify the reason.

Circumstances:
During the takeoff roll at Fukuoka-Itazuke Airport runway 16, at a speed of 158 knots, the captain started the rotation. During initial climb, at a height of about 3 metres, the right engine suffered a loss of power after a fan blade located on the 1st stage of the high pressure compressor disk separated. The N1 dropped to 23,7% and five seconds later, the flight engineer informed the crew about the failure of the engine n°1. The captain decided to abort the takeoff and landed back on runway. The aircraft contacted ground with a vertical acceleration of 2,1 g then thrust reversers were deployed and ground spoilers were extended. Unable to stop within the remaining distance, the aircraft overran, crossed a road, skidded for about 620 metres before coming to rest in an open field, bursting into flames. Three passengers were killed.
Probable cause:
Although the CAS was well in excess of V1 and the aircraft had already lifted off from the runway, the takeoff was aborted. Consequently the aircraft departed the end of the runway, came to rest and caught fire. It is estimated that contributing to the rejection of the takeoff under this circumstance was the fact that the CAP's judgement in the event of the engine failure was inadequate. Investigation revealed that the turbine blade that failed, had operated for 30913 hours and 6182 cycles. General Electric had advised customers to discard blades after about 6000 cycles.

While cruising at a relative low altitude under VFR mode in IMC conditions (snow falls), the crew saw a mountain. To avoid a collision, the pilot increased engine power but the aircraft struck trees and crashed in a wooded area located on the Takami Peak, near Kawatana. Both pilots were injured and the aircraft was destroyed.

Three student pilots and one instructor were on board this Crusader to complete a training mission. While in cruising altitude under VFR mode over the north part of the Ariake Sea, the pilot-in-command lost control of the aircraft that entered a dive and crashed 3,6 km south of the Suminoe harbor. The aircraft disintegrated on impact and all four occupants were killed.
Probable cause:
It is believed that the loss of control occurred during a VFR training flight while the crew was performing emergency procedures. The crew was unable to recover the aircraft.

Shortly after takeoff, while climbing, the crew encountered poor visibility and low clouds when the airplane struck the slope of Mt Tachizawa located south of Fukuoka. The aircraft was destroyed upon impact and all five occupants were killed.
