Circumstances:

After a night takeoff from Rochester Airport in poor weather conditions, the airplane suffered an engine failure. The pilot lost control of the aircraft that crashed in flames past the runway end. The aircraft was destroyed and all four occupants were killed.

Probable cause:

Powerplant failure for undetermined reasons and stall spin during initial climb. The following contributing factors were reported:
- Improper operation of flight controls,
- Failed to maintain flying speed,
- Misused or failed to use flaps,
- Low ceiling,
- Snow and blowing snow,
- Visibility 1/2 mile or less,
- Flaps full down.

On takeoff at Angoulême-Brie-Champniers Airport, the twin engine airplane went out of control and crashed for unknown reasons. The aircraft was destroyed and the pilot, sole on board was killed. He was flying back to his base in Antwerp after dropping passengers flying from Antwerp.

While in initial approach to Bennington-William H. Morse State Airport, the twin engine airplane lost height, stalled and crashed in flames in Argyle, about 26 miles northwest of Bennington Airport. The aircraft was destroyed and the pilot, sole on board, was killed.

Probable cause:

Loss of control after the pilot failed to maintain flying speed. The following contributing factors were reported:
- Lack of familiarity with aircraft,
- Attempted operation beyond experience/ability level,
- Icing conditions including sleet, freezing rain,
- Fog,
- After four day checkout in an aircraft, instructor told the student pilot not to attempt IFR flight until he attended formal school.

Circumstances:

The airplane was completing a charter flight from Denver to Granby on behalf of Conquest Aero Resources. While cruising in poor weather conditions (low clouds and snow falls), the pilot failed to realize his altitude was insufficient when the airplane struck a mountain located near Rollinsville. The wreckage was found two days later, on December 28, and all nine occupants have been killed.

Probable cause:

Controlled flight into terrain after the pilot continued VFR flight into adverse weather conditions. The following contributing factors were reported:
- Improper in-flight decisions,
- Disregard of good operating practice,
- High obstructions,
- Low ceiling,
- Snow,
- Fog.
