Circumstances:
Shortly after takeoff from Dallas-Love Field Airport, while climbing, the pilot lost control of the airplane that stalled and crashed in flames in an open field. The aircraft was destroyed and all three occupants were killed.
Probable cause:
Failure an separation in flight of a propeller blade during initial climb due to fatigue fracture. It was determined that the right engine Hartzell propeller blade separated at shank radius. Crankshaft counterweight liners excessively worn.

Circumstances:
Shortly after takeoff from Red Bank-Schrewsbury Airport, while climbing in foggy conditions, the left engine failed. The pilot lost control of the airplane that stalled and crashed in a field. All nine occupants were killed. They were on their way to New York-John Fitzgerald Kennedy Airport in this early morning flight.
Probable cause:
Failure of the left engine during initial climb for undetermined reason. However, weather conditions were conducive to carburetor icing. The pilot failed to maintain flying speed and to follow emergency procedures. At the time of the accident, the aircraft was near max gross weight and after CofG limit.

While cruising at an altitude of 8,000 feet, the pilot encountered severe icing conditions and contacted ATC to obtain the permission to climb to 10,000 feet. Shortly later, the twin engine aircraft went out of control, entered a dive and eventually crashed in a field located in Heflin. Both occupants were killed.

Loss of control in icing conditions for undetermined reason.

During the takeoff roll, after a course of 1,058 feet, the undercarriage were retracted. The aircraft sank on runway and both engines struck the ground. Out of control, the aircraft crashed in flames, killing all five occupants.
Probable cause:
Premature gear retraction on part of the crew who failed to abort takeoff. The aircraft needed more than 1,525 feet ground roll for takeoff and the props hit the runway surface 1,058 feet from start of roll.

While approaching Mithapur from the north, the twin engine aircraft was shot down by the pilot of a Pakistan Air Force F-86 Sabre, dove into the ground and crashed in a huge explosion in a field located in Suthri. The aircraft was totally destroyed and all seven occupants were killed, among them Balwantrai Mehta, Chief Minister of the Gujarat State who was flying with his wife. At the time of the accident, the airplane was off course for undetermined reason but Indian Authorities reported the aircraft did not penetrate the Pakistan Airspace.
Probable cause:
Shot down by the pilot of a Pakistan Air Force fighter.

Probable cause:
It was determined that both engines failed due to a fuel starvation. This was caused by a mismanagement of fuel and inattentive to fuel supply on part of the pilot.
