Circumstances:
The crew departed Moscow-Sheremetyevo Airport on a positioning flight to Kharkiv where some passengers should be picked up for a charter flight to Lyon. On final approach to Kharkiv-Osnova Airport runway 07, at a distance of 4,600 metres from runway threshold, the crew selected flaps down. The right flap deployed to an angle of 20° (as requested by the crew) while the left flap deployed to an angle of 6°. This asymmetry caused the aircraft to roll to the left then to enter an uncontrolled descent before it crashed on the shore of lake Komsomolsk. The aircraft disintegrated on impact and debris were found 140 metres further while the main wreckage sank in the lake, some 4,300 metres from the runway threshold and 750 metres to the left of its extended centerline. All three crew members were killed.
Probable cause:
The crew lost control of the aircraft after they selected flaps down. Investigations showed that a technical failure occurred on the flap deployment synchronization system due to the failure of a drive pinion.

The crew was cleared to reduce his altitude and to divert to Kharkiv Airport. On descent, the engine n°1 and the left wing partially detached due to intense fire. Out of control, the airplane nosed down to an angle of 90°, overturned and crashed in an open field located 6,5 west of Rakitnoye, about 31 km south of Kharkiv Airport. The aircraft was totally destroyed and all nine occupants were killed.
Probable cause:
The engine n°1 caught fire in flight after a fuel line ruptured for unknown reasons.

Circumstances:
After takeoff from Kharkiv, the crew informed ATC that the left engine caught fire and was cleared to divert to the Sokolnikovo Airfield deserving Factory #135 near Kharkiv. On approach, the left engine exploded and the captain decided to attempt an emergency landing in a wasteland near the airport. The aircraft belly landed and slid for few dozen meters before coming to rest in flames. All seven occupants escaped uninjured while the aircraft was partially destroyed by fire.
Probable cause:
Fire and explosion of the left engine following the rupture of a fuel line after takeoff, which led the fuel to leak in the engine compartment and to catch fire while contacting high temperature elements.

Circumstances:
The crew started the approach to Kharkiv Airport by night and a limited visibility. He was cleared to descend by ATC when, at a speed of 270 km/h, the airplane struck tree tops, stalled and crashed on the slope of a wooded hill located 12 km short of runway threshold. A crew member and 18 passengers were killed while 31 other occupants were injured, some of them seriously. The aircraft was destroyed.
Probable cause:
It was determined that the accident was the consequence of ATC error who transmitted to the crew a wrong barometric pressure of 757 mm instead of 737 mm, causing the aircraft to descend 220 meters too low. The crew was also blamed as he failed to react to the warning horn coupled to the ground proximity warning system that sounded for 23 second prior to impact.

Circumstances:
Following an uneventful flight from Vnukovo Airport at an altitude of 7,200 meters, the crew was cleared to approach Kharkiv-Osnova Airport and to descent to 1,500 meters. While approaching at a speed of 400-500 km/h with a rate of descent of 6 meters per second, the airplane lost its both wings, dove into the ground and crashed in a wooded area located in Rus'ka Lozova, about 25 km north of Kharkiv-Osnova Airport. The aircraft was totally destroyed and all 122 occupants were killed.
Probable cause:
It was determined that a structural failure of the center section lower panel caused the failure of both wings due to fatigue cracks. The rate of descent and the presence of atmospheric turbulences were considered as contributing factors. Investigations revealed that this portion of the aircraft could not be inspected by technicians during a maintenance process due to a faulty conception of the aircraft, thus the fatigue fractures could not be detected in time. Following this tragedy, the Antonov AN-10 would not be operated anymore in passenger service.

Circumstances:
While completing a crop-spraying flight from the airstrip of the sovkhoz (state farm) "Udarna Pratsya" in the Shevchenkovo district of the Kharkov region, the inexperienced captain made a sharp turn to the left at low height when control was lost. The airplane crashed in a field located 2 km northwest of Slobodovka. Both pilots were killed and the aircraft was destroyed.
Probable cause:
It was determined that the pilot-in-command was inexperienced on this type of aircraft with only 87 flying hours. The accident was the result of errors on part of the pilot-in-command. Poor flight preparation and an insufficient period of rest time were considered as contributing factors.
