The approach and landing at Tashkent-Yuzhny Airport were completed in marginal weather conditions with strong crosswinds. After touchdown, the airplane went out of control, veered off runway and came to rest. All 8 crew and 89 passengers escaped unhurt while the aircraft was damaged beyond repair.

Three minutes after its takeoff from Petrolina Airport, the airplane was seen to initiate a turn to the left when it stalled and crashed in a field. The airplane caught fire and was almost destroyed. Both pilots were killed while four passengers were injured. A fifth passenger escaped uninjured.

After takeoff from Yopal Airport, while in initial climb, the left engine failed. The airplane rolled to the left, causing the left wingtip to struck the ground. Out of control, the airplane cartwheeled and crashed in flames. All 36 occupants were killed.

Circumstances:
While descending to Newport News-Williamsburg Airport on a schedule flight from Washington DC, the right engine failed. The crew lost control of the airplane that entered a dive and crashed into the sea off Hampton, about 12,5 miles southeast of the airport. The aircraft was lost and all four occupants were killed (the copilot's body was never recovered).
Probable cause:
Uncontrolled descent during initial approach due to engine structure failure (valve assemblies). The following contributing factors were reported:
- Diverted attention from operation of aircraft,
- Electrical system: generators/alternators,
- Failed to use or incorrectly used miscellaneous equipment,
- Aircraft came to rest in water,
- N°5 cylinder on right engine, exhaust valve would not seat,
- Right engine alternator bracket failed.

Shortly after takeoff from runway 27 at Bombay-Santa Cruz Airport, while climbing, the captain contacted ATC and declared an emergency after the right engine exploded and caught fire. The crew was cleared to return for an approach to runway 09 and initiated a circuit. On final approach, at a height of 300 feet, the airplane entered a nose down angle of 45° then crashed in a huge explosion about one km short of runway 09. The aircraft disintegrated on impact and all 95 occupants have been killed.
Probable cause:
Failure of the 10th stage of the compressor disc, probably after a blade failed due to fatigue cracks. Debris sectioned fuel and hydraulic lines, causing the fluid to leak in the engine and to ignite while in contact with high temperature elements. The fire quickly spread as the crew failed to shut off the fuel supply system.

The four engine airplane departed Bridgetown-Grantley Adams Airport at 1715LT on a schedule flight to Havana via Kingston, Jamaica. Nine minutes later, the captain declared an emergency and informed ATC about an explosion on board. He was cleared to return for an emergency landing when shortly later, the airplane entered a right turn, lost height and plunged into the sea about 8 km offshore. The airplane disintegrated on impact and all 73 occupants were killed, among them 24 members of the Cuban National Fencing Team.
Probable cause:
It was determined that an explosive device has been placed on board the aircraft, at the rear part of the cabin, and that exploded during initial climb, causing severe damages to control systems. The terrorist act was claimed by the Coordination of United Revolutionary Organizations, an anti-Castro organization, with Luis Posada Carriles at his head.
