Circumstances:
After liftoff from Port Clinton Airport, while on a touristic flight to Put-in-Bay, the right engine lost power. The airplane lost height and crashed by the runway. All 16 occupants were evacuated, a passenger was seriously injured.
Probable cause:
Engine failure or malfunction and stall during initial climb due to mixture control assemblies failure. The following factors were reported:
- Powerplant improperly secured and installed,
- Inadequate maintenance and inspection,
- Pilot failed to maintain flying speed,
- Right engine lost power,
- Safety wire failed, allowed mixture control to move from rich position,
- Wires in poor condition.

Circumstances:
The crew was completing a training mission from Pittsburgh to Wheeling. While cruising at an altitude of 5,000 feet, the crew decided to perform several stalls when control was lost. The airplane entered a dive and crashed in flames in an open field located near Wellsburg. The aircraft was totally destroyed and all three occupants were killed.
Probable cause:
Loss of control in stall maneuver for unknown reason. Control not recovered before impact.

Circumstances:
Shortly after takeoff from Cleveland-Hardwick Airport, while in initial climb, the right engine lost power. The pilot was cleared to return for an emergency landing and started an immediate turn before trying to feather the right propeller. Due to insufficient speed, the airplane lost height, stalled and crashed, killing all five occupants.
Probable cause:
Loss of power on the right engine during initial climb due to material failure. The following factors were reported:
- Powerplant failure due to master and connecting rods failure,
- The pilot failed to follow the approved procedures,
- The pilot failed to maintain flying speed,
- Improper emergency procedures,
- Right engine n°5 cylinder connecting rod bolt failed,
- The pilot tried to return to airport before feathering the right propeller,
- Aircraft near gross weight.

Shortly after takeoff from Point Mugu NAS, while in initial climb, the right engine lost power and caught fire. The crew elected to return for an emergency landing when control was lost and the airplane crash landed by the runway and came to rest. All three occupants were slightly injured and the aircraft was damaged beyond repair. For unknown reason, the fire extinguishing system was inoperative.
Probable cause:
Partial loss of power on one engine during initial climb due to material failure. The following factors were reported:
- Failure of the fuel system,
- Failure of the fire extinguishing system,
- Engine fire,
- Component failure of right engine fuel feed system,
- Failure of hydraulic line.

Circumstances:
En route from Laredo to Dallas, one of the engine lost power due to oil exhaustion. The pilot was cleared to divert to Austin-Robert Mueller Airport but following a wrong approach configuration, the aircraft was not properly aligned. After touchdown on a wet runway, the airplane suffered hydroplaning then veered off runway and came to rest. All three occupants were uninjured while the aircraft was damaged beyond repair.
Probable cause:
In-flight engine failure or malfunction. The following factors were reported:
- Inadequate maintenance and inspection,
- Oil exhaustion, engine lubrication system,
- Improper IFR operation,
- Failed to initiate a go-around,
- Lack of familiarity with aircraft,
