Circumstances:
While approaching Vitoria da Conquista Airport runway 15, the crew failed to realize his altitude was too low. On short final, the aircraft impacted a small mound located few metres short of runway threshold. On impact, both main landing gears were torn off. The aircraft slid on runway for about 300 metres then veered off runway to the left and came to rest in a grassy area some 35 metres left of the runway with the right engine on fire. All 38 occupants escaped uninjured while the aircraft was damaged beyond repair.
Probable cause:
The following factors were identified:
- During the approach, the flight crew's attention was focused on the characteristics of the geographical relief and presence of birds, reducing their awareness as to the maintenance of the approach slope.
- The group culture of maintaining a low angle of approach led the crew to choose the runway aspect instead of the VASIS as a reference for the approach, making them susceptible to various types of spatial illusion.
- Taking into account copilot's report that he was not succeeding in correcting the aircraft glide path relative to the runway, one may suppose that he was not applying the appropriate amplitude for such correction.
- The physical characteristics of the runway 15 (the active one) contributed to a wrong perception of the ideal glide path. The pronounced acclivity of the runway, its width (narrower than the runways on which the crew was accustomed to operate), and the low terrain near the threshold, caused in the pilots a perception that they were above the ideal approach slope, leading them to seek correction, which resulted in an angle of approach below the ideal one.
- For the flight in question, the company chose two pilots who had never operated in SBQV. A crewmember with previous experience in the locality would have a higher level of awareness in relation to the specific characteristics of the aerodrome.
- No company publications were found that could provide the pilots with guidance on the specifics of SBQV, capable of helping with the management of the risks associated with the operation in that aerodrome.

Shortly after takeoff from Vitória da Conquista Airport, the twin engine airplane encountered difficulties to gain sufficient height. During initial climb, it stalled and crashed about 2 km from the runway end. Both pilots were killed.
Probable cause:
The exact cause of the accident could not be determined. However, it is believed that the captain elected to make a low pass over the house of a friend when he lost control of the aircraft.

On approach to Vitória da Conquista Airport, the crew lowered the gear. As it failed to lock, the captain decided to make a go-around, increased power on both engines and completed a low pass over the runway before joining the circuit. As he completed a turn to the left, the aircraft hit a telephone pole, stalled and crashed in flames near the airport. Four passengers and a crew member were killed while 16 other people were injured. The aircraft was destroyed.
Probable cause:
It was determined that the accident was caused by the failure of the pilot-in-command to maintain a correct attitude whilst the inspection was being made to ascertain the position of the landing gear. It was also reported that the following factors contributed to the accident:
- Error of judgement on the part of the pilot in opening the throttles for a go-around with the gear extended,
- Failure of the landing gear locking mechanism.

The crew was performing a cargo flight from Vitória to Rio de Janeiro. The three engine aircraft was spotted in poor weather conditions between the cities of Araruama, Saquarema and Maricá, between 1600 and 1700LT. It then disappeared in unknown circumstances. No trace of the aircraft nor the crew was found. It is possible the aircraft crashed into the Ocean off Maricá.

The crew started the approach to Santos Dumont Airport in poor weather conditions. While flying at an insufficient height of 180 feet in low visibility, the aircraft hit a hill and crashed in Gramacho, about 20 km north of Santos Dumont Airfield. A pilot and a passenger were killed while 17 other occupants were injured.
Probable cause:
The crew decided to complete the approach under visual flight rules while poor weather conditions necessitated an approach under instruments flight rules. Due to low visibility, the aircraft was approaching at the insufficient height of 180 feet while the minimum descent altitude was fixed at 1,200 feet.

During the takeoff roll, the left engine suffered vibrations and the captain decided to abandon the takeoff. In a certain confusion, the copilot inadvertently retracted the undercarriage, causing the aircraft to sink on its belly. It skidded for several yards before coming to rest. While the airplane was damaged beyond repair, there were no casualties.
Probable cause:
Vibrations on the left engine during takeoff caused by the failure of a propeller hub. Poor crew coordination and communication, making the copilot to retract the undercarriage while the aircraft was still rolling for takeoff.

On approach to Rio de Janeiro following a cargo flight from Vitória, the crew encountered technical problems and was forced to lower the gear manually. Shortly later, both engines lost power and the captain decided to divert to Galeão Airport closer to his position than Santos Dumont. During the last turn, the aircraft stalled and crashed into the Guanabara Bay. All three crew members were injured while the aircraft was lost.
Probable cause:
It was determined that the loss of power on both engines was caused by the failure of the hydraulic system.

En route from Vitória da Conquista to Salvador, the crew encountered poor weather conditions and the captain decided to reduce his altitude to avoid clouds when the aircraft went through a cumulonimbus area. Severe turbulences were encountered and the aircraft went out of control when both wings broke off and separated. The aircraft dove into the ground and crashed in the region of Ilhéus. Two passengers were seriously injured while 13 other occupants were killed.
Probable cause:
It was reported that the aircraft encountered severe turbulences while cruising in a cumulonimbus area, which caused the rupture of both wings that were overstressed (above wing certification). The decision of the crew to start a rapid descent and to fly into a cumulonimbus area was imprudent.
