Mombasa

On October 11, 2019 at about 0902 Kenya daylight time, a Silverstone Air Services Fokker 27 Mark 050 registration 5Y-IZO experienced runway excursion on takeoff runway 14 at Wilson Airport with 55 occupants onboard (5 crew and 50 passengers). The aircraft was performing a scheduled flight from Wilson airport to Mombasa, Lamu and back to Wilson airport. The aircraft was fueled then the passengers boarded the plane before the normal pre take off procedures which were reported as uneventful as collaborated by the information from the FDR. The Right Hand engine receded the Left Hand engine in startup as depicted by the parameters from the recorders. From engine startup, taxi, lineup runway 14, power up and including the initial stages of ground roll were uneventful. At 06.02.50 GMT, while still on ground roll take off, the LH engine ITT began to fluctuate, dropped from 707.8 to 175.8. The RH ITT remained stable. The other parameters of both engines were stable. Subsequent power down ensued at 06.03.01, IAS dropped from 110.2Knots and the aircraft magnetic heading increased gradually to 144 consistent with the deviation of the aircraft to the right of the runway axis when it left the paved surface of the runway, before colliding with the airport perimeter fence, went through an embankment before coming to rest tilted to the right after the starboard wing made contact with a tree. The passengers were deplaned through the two left hand side front and rear exit doors. Immediately the event occurred, ATS Wilson airport issued a crash alert and Wilson Airport Fire Service (AFS) responded without delay, found the passengers already evacuated from the aircraft and applied foam to the sections of the aircraft with leaking fuel. Runway 14/32 was closed and all inbound and departure traffic made use of the remaining runway. Preliminary information retrieved from the Flight Data Recorder Radar regarding the engine ITT seems to correlate with the flight crew statements regarding the observed fluctuations on the LHS engine ITT.

On final approach to Mombasa-Moi Airport, the aircraft was too low and collided with approach lights. The crew continued the approach and the aircraft landed safely. Few seconds later, while evacuating the runway to the taxiway, the right main gear collapsed. All three crew members escaped uninjured while the aircraft was damaged beyond repair.

After takeoff from Musiara Airport, during initial climb, the crew raised the landing gear and flaps. About 45 seconds after liftoff, the left engine failed. The crew elected to restart it but without success and decided to divert to Olkiombo Airport located 18 km from their position. On approach to runway 09, the aircraft stalled and crashed in an open field. The copilot was killed while 19 other occupants were injured, some seriously. The aircraft was destroyed.

The four engine aircraft was completing a cargo flight from Mwanza to Ostend with intermediate stops in Mombasa and Cairo, carrying six crew members and a load of 34 tons of fish. After takeoff from runway 03, the aircraft collided with approach lights and an earth mound then crashed few hundred metres past the runway end, bursting into flames. The aircraft was totally destroyed and all six occupants were killed. At the time of the accident, the first portion of 2,600 feet of runway 03 which is 10,991 feet long were not available due to work in progress. It was reported that the total weight of the aircraft was 135 tons.

Few minutes after takeoff from Mombasa-Daniel Arap Moi Airport, while climbing, the twin engine aircraft went out of control and crashed in an open field located 20 km south of the airport. Both passengers were seriously injured and the pilot was killed.

En route from Mombasa to Mogadishu, while in cruising altitude over the Indian Ocean, the crew contacted ATC and declared an emergency following an explosion in the cargo compartment. The crew initiated an emergency descent and elected to divert to the nearest airport when control was lost. The aircraft crashed in the sea about 200 metres offshore, some 7 km from Malindi. Six crew members were rescued while eight others were killed.

En route from Mombasa to Kichwa Tembo, the pilot was informed that a DC-3 suffered a mishap upon landing at Musiara Airstrip and apparently decided to make a low pass to show the scene to the passengers. While approaching the airfield at an altitude of 250 feet, the aircraft collided with a white-backed vulture who went through the windshield and killed the pilot instantly. Control was lost and the aircraft crashed in an open field, bursting into flames. All nine occupants were killed.

Just after liftoff, while climbing to a height of about 5-10 meters, the left engine failed. The aircraft banked left, causing the left wing to struck the runway surface. Out of control, the aircraft crashed next to the runway and came to rest. All nine occupants were injured.

On approach to Mombasa-Moi Airport while on a positioning flight, both engines failed simultaneously. The crew attempted an emergency landing when the aircraft struck trees and crash landed near a road about 18 km from the airport. Both pilots were slightly injured and the aircraft was damaged beyond repair.

On a charter flight from Mombasa to Nairobi, the pilot encountered foggy conditions. While cruising at an insufficient altitude, the twin engine aircraft struck the Malwa Hill located near Machakos. The aircraft was destroyed and all nine occupants were killed, among them eight European tourists; 4 Italians, two French and two Swiss.
