Circumstances:
During the takeoff roll at Dundo Airport, the twin engine aircraft deviated to the left after the crew encountered directional control problems. Few seconds later, at a speed of 190 km/h, the aircraft deviated to the right then veered off runway. On rough ground, the aircraft became airborne the struck power cables, stalled and crashed. All six crew members were injured and the aircraft was destroyed.

On final approach to Huambo-Albano Machado Airport, the crew realized that a vehicle was on the runway. Instead of initiating a go-around procedure, the crew landed after the vehicle, too far down a wet runway. Unable to stop within the remaining distance, the aircraft overran and collided with a concrete wall. All 10 occupants escaped uninjured and the aircraft was damaged beyond repair.

On final approach to M'Banza Kongo Airport, the aircraft was too low and struck the ground 3,200 metres short of runway. It reached a little altitude then descended again and struck the ground a second time two metres short of runway. On impact, the left main gear struck a drainage ditch and was torn off. The aircraft slid on the ground, veered to the right when the right wing struck a bus. All three crew members escaped with minor injuries while seven people in the bus were killed.
Probable cause:
The M'Banza Kongo runway is relatively short and the crew must complete a low approach to be able to land as early as possible. Here, the crew completed a too low approach, causing the aircraft to struck the ground twice before runway threshold.

En route to Cabinda on a cargo flight, the pilot reported problems with the right engine. He elected to make an emergency landing when the aircraft crash landed in an open field located 16 km from Cabinda Airport. The aircraft was damaged beyond repair and the pilot escaped uninjured. At the time of the accident, weather conditions were poor.
Probable cause:
Engine problems for unknown reasons.

The four engine aircraft leased from Carib Air Transport to Angola Air Charter was completing a cargo flight from Luanda to Cafunfo, carrying six passengers, four crew members and a load of various goods for the Luanda Norte Province. Shortly after takeoff from runway 24, while climbing to a height of about 50-60 meters, the aircraft banked right then to the left. It stalled and struck the ground on the right side of the runway and disintegrated. All 10 occupants were killed.
Probable cause:
The investigation into the cause of the crash was severely hampered by the fact that the wreckage had been moved from the scene by the orders of an Air Force official, without authorization of the civil aviation authorities (DNAC). Additionally, both flight recorders were not located in the wreckage. The commission found several pieces of debris along the right side of the runway, 300-400 meters from the crash site.
"The accident is deduced to have originated with a probable internal explosion of an unidentified foreign object that perhaps had as its site of action an area of the fuel storage area on the right wing of the aircraft. When the explosion occurred, it would have caused the craft to dip to the right owing to the lack of support on the aerodynamic planes on that side and then to fall to angle of more or less 45 degrees with runway 24 due to asymmetry in the weight of that wing. Greater weight on the [left] side' may have caused the aircraft to stall, because the first impact actually was made by the tip of the left wing striking the ground, prior to the fire that destroyed it totally."

On final approach to Techamutete Airport, the aircraft was shot down by a SAM-7 surface-to-air missile and crashed in a huge explosion few hundred meters short of runway. The aircraft was totally destroyed and all 29 occupants were killed, among them Brigadier General Francisco Cruz Bourzac, deputy minister for Armament in the Cuban Government.
Probable cause:
It was determined that the aircraft was mistakenly shot down by Cuban soldiers based near the Techamutete Airport who thought this was an enemy airplane.

On final approach to Luanda-4 de Fevereiro Airport, the crew failed to realize his altitude was too low when the aircraft struck a radio antenna located on the top of a building. Hydraulic and control lines were damaged by the collision. After touchdown, the aircraft was unable to stop within the remaining distance, overran, lost its nose gear and came to rest. All nine occupants escaped uninjured while the aircraft was damaged beyond repair.
Probable cause:
The aircraft was unable to be stopped within the available distance due to hydraulic problems sustained after the collision with the radio antenna on approach. Poor planned approach on part of the flying crew who continued the approach below the glide.

After takeoff from Huambo-Albano Machado Airport, while climbing, an explosion occurred in the rear of the cabin. The pilot declared an emergency and was cleared to return. Due to the partial failure of the hydraulic system, the crew was unable to lower the flaps and the aircraft landed at high speed. After touchdown, the aircraft was unable to stop within the remaining distance, overran and lost its undercarriage and both engines before coming to rest 180 meters further. All 142 occupants were evacuated and 15 of them were injured. The aircraft was destroyed.
Probable cause:
It was determined that a bomb exploded in the rear of the cabin during initial climb. It was placed on board by UNITA rebels.
