Circumstances:
The four engine aircraft landed too far down a wet runway at Iquitos-Coronel Francisco Secada Vignetta Airport. After touchdown, the crew started the braking procedure but the aircraft was unable to stop within the remaining distance. It overran, lost its undercarriage and came to rest few dozen meters further. All four crew members evacuated safely while the aircraft was damaged beyond repair.
Probable cause:
Wrong approach configuration on part of the crew who landed the aircraft too far down the runway, reducing the landing distance available. At the time of the accident, the runway was wet, which was considered as a contributing factor.

Circumstances:
The twin engine airplane departed San Ramón-Capitan Alvarino Airport at 1730LT on the second leg of the trip to Lima. On board were 57 passengers, a crew of five and a load of various goods including wood and animals. While climbing, the captain informed ATC about engine problems and that he was unable to maintain the assigned altitude. Ten minutes after takeoff, the aircraft struck the slope of a mountain located about 30 km northwest of Tarma and disintegrated on impact. All 62 occupants were killed.
Probable cause:
It was determined that there were 57 passengers on board while the aircraft was certified for 44 passengers max. It was also determined that the aircraft was unable to gain sufficient altitude because it was overloaded and the crew could not obtain full engine power as they were submitted to strong contribution.

The approach and landing in Iquitos was completed in heavy rain falls. After landing on a wet runway, the aircraft was unable to stop within the remaining distance, overran and came to rest with the right engine torn off. All 139 occupants were evacuated, among them 14 passengers were injured. The aircraft was damaged beyond repair.
