Circumstances:
The single engine airplane departed San Lorenzo at 0900LT on a flight to Tarapoto, carrying 10 passengers and two pilots. About 15 minutes into the flight, the crew encountered technical problems with the engine and attempted an emergency landing when the aircraft crash landed in a wooded area located some 15 km southeast of San Lorenzo. All 12 occupants were rescued and the aircraft was damaged beyond repair.

Circumstances:
During the takeoff roll from runway 31 at El Estrecho Airport, the crew encountered an unexpected situation and rejected takeoff. Unable to stop within the remaining distance, the aircraft overran, crossed a road and crashed against a wooded embankment. The right wing was partially sheared off and the undercarriage were torn off. All 17 occupants were injured, five seriously. One passenger died from injuries sustained.

Circumstances:
After touchdown on runway 06 at Iquitos-Coronel Francisco Secada Vignetta Airport, the aircraft went out of control and veered off runway to the left. It contacted trees and bushes, lost its right wing and came to rest 100 metres to the left of the runway, broken in two. A fire erupted but was quickly extinguished. All four crew members were slightly injured and the aircraft was destroyed.
Probable cause:
Runway Excursion due to loss of directional control of the aircraft after landing by disengaging the propeller stops for aerodynamic braking at a higher than established speed, which generated an uncontrollable leftward turning moment of the trajectory, due to asymmetric thrust of the propellers.
The following contributing factors were identified:
- Performing a landing on an Unstabilised Approach, not executing the established Go-Around procedure.
- Performing a failed landing due to asymmetric thrust of the propellers, not applying the procedure established in the Operator's QRH to shut down the failed engine.
- Probable failure of the KTA Fuel Regulator of the Automatic Engine Rotation Frequency and Power Regulation System, which blocked the reduction of the power of the N°2 engine to established operating regimes for the approach and landing.

The seaplane landed hard in the Caballococha's Laguna. Upon landing on water, the right float was torn off, the aircraft overturned and sank. All 11 occupants were rescued and the aircraft was damaged beyond repair. It was performing a flight from Iquitos on behalf of the 42nd Group of the Peruvian Air Force.

Circumstances:
The aircraft was completing a charter flight from Iquitos to Andoas on behalf of the Occidental Petroleum Company, carrying eight crew members and 80 employees of this company and their subcontractors. While descending to Andoas-Alferez FAP Alfredo Vladimir Sara Bauer Airport by night, the crew encountered poor weather conditions with rains falls and limited visibility. On final, the aircraft descended below the glide, collided with trees and crashed in a dense wooded area located 5 km short of runway 12. First rescuers arrived on site three hours later. Thirteen occupants included two crew members were seriously injured while 75 other occupants were killed.
Probable cause:
For undetermined reasons, the crew descended below the minimum safe altitude during an approach in IMC conditions until the aircraft impacted the ground.
