The airplane departed RAF Wattisham at 2100LT on September 4 on an operation 'Mardiek'. Upon landing at 0305LT, it went out of control and crashed with a parked aeroplane. Two crew members were killed and a third was injured.

The airplane departed RAF Stradishall on the evening of August 30 on a bombing mission over Berlin. The following morning, while returning to base, low on fuel, it undershot the runway, ran into a ditch, lost its undercarriage and came to rest. All crew members evacuated safely.

The airplane departed RAF Wattisham at 2140LT on August 30 on an operation over Emden. It crashed at 0220LT at Haskell's Farm in Offton, some 11 km northwest of Ipswich. Two crew were killed and a third was injured.

The airplane departed RAF West Raynham at 2205LT on 19 August to attack Antwerp. While returning to his base, the crew was forced to ditch the airplane off Lowestoft. A crew was drowned and two others were rescued.

The airplane departed RAF Mildenhall at 2146LT on August 11 on a bombing mission over Gelsenkirchen. While returning at night and making an approach wide of the flarepath, the bomber collided with a radio mast and crashed at Beck Row. All six crew members were killed.

The airplane departed RAF Feltwell on an operation to Horst, northwest of Hamburg. At night, it crash landed in Barton Mills, smashing into a ridge. The pilot was killed and five other occupants were injured.

The airplane departed RAF Wattisham at 0640LT to attack Soesterberg Airfield. Damaged by the German Flak, it was returning to its base when it crashed upon landing. The pilot was injured and both other occupants escaped unhurt.

The airplane departed Vitry-en-Artois on a reconnaissance mission. While approaching the British coast, it was shot down by a Spitfire and crashed into the sea some 30 km off Lowestoft. All five crew members were killed.

The airplane departed RAF Newmarket at 2150LT on July 25 on a raid over Germany. In the middle of the night, while approaching RAF Newmarket with failing engines, the pilot lost sight of the runway in a sudden rain squall, causing the aircraft to collide with trees located in the airfield boundary and to crash. All six crew members were rescued.

The crew departed RAF Honington on a local night training flight. While flying in the vicinity of the airbase, the aircraft crashed in unknown circumstances in a field located in Troston, about three km southwest of the airfield, bursting into flames. Both crew members were killed.
