Circumstances:
The airplane departed RAF Linton-on-Ouse at 2235LT on April 15 on an operation to Kiel. Over the target area, it was hit by enemy fire and both left engines were damaged. While returning to base the following night, the airplane crash landed against a tree and came to rest at Tollerton, near the airfield. All seven crew members were rescued, among them two were slightly injured.

The crew was completing a local training flight at RAF Church Fenton. While circling on approach, an engine failed, followed shortly later by a loss of power on the second engine. The airplane stalled and crash landed. All three crew members escaped uninjured.

The airplane departed RAF Leeming at 2021LT on March 18 on an operation to Kiel. While returning to base the following night, flying at an altitude of 6,000 feet, the left engine failed and caught fire. The flames spread to the fuselage and the crew were ordered to bail out. Four occupants were found uninjured while the pilot was overcome by smoke and his body was found in the wreckage.
