Circumstances:
After takeoff from Guryev Airport, while climbing to an altitude of 900 metres, the right engine lost power and flamed out. The crew shut down the engine and feathered the propeller before completing a circuit to return to the airport. On final, the crew encountered icing conditions and as the aircraft was not properly aligned, the captain decided to initiate a go-around when the aircraft lost height and crashed in a field with its undercarriage partially retracted. The aircraft slid for 418 metres before coming to rest 6 km from the airport. All 51 occupants evacuated safely while the aircraft was damaged beyond repair.
Probable cause:
Failure of the right engine during initial climb for unknown reasons. Icing conditions were considered as contributing factor.

Circumstances:
Few dozen minutes after takeoff from Guryev Airport, while in cruising altitude, the crew noticed a higher than normal fuel consumption and realized he could not reach the destination. The captain decided to fly back to Guryev but a the fuel remaining was insufficient, the crew attempted an emergency landing in an open field located about 42 km from Guryev Airport. The aircraft crash landed in a field and came to rest. All six occupants escaped uninjured while the aircraft was damaged beyond repair.
Probable cause:
It was determined that the ground personnel in charge of the refuel operations at Guryev Airport failed to replace the fuel tank cap properly. This caused the fuel to leak out of the tank during flight, forcing the crew to attempt an emergency landing. The failure of the crew to check the aircraft prior to takeoff according to published procedures was considered as a contributing factor.

Aircraft flight hours:
1281
Circumstances:
The airplane departed Uralsk on a cargo flight to Guryev, carrying two crew members and a load of 400 kg of fret and mail. En route, weather conditions worsened. Due to poor visibility, the pilot made an emergency landing near the village of Kalmykovo. About an hour later, as the visibility seems to be better, he took off again. The airplane failed to arrive at destination. The wreckage was found near the village of Orlik. The aircraft was destroyed and both occupants were killed.
Probable cause:
The following factors were identified:
- The pilot decided to continue the flight in poor visibility (between 50 and 200 metres) and failed to return,
- Lack of discipline,
- Overconfidence,
- The weather bulletin transmitted to the crew prior to departure from Uralsk were incomplete and inaccurate.
