Circumstances:
On final approach to Barcelos Airport, the crew encountered poor weather conditions with reduced visibility due to heavy rain falls. On short final, the crew decided to initiate a go around procedure when the airplane apparently stalled and crashed on an embankment located near runway 09/27. The airplane was destroyed and all 14 occupants were killed. All 12 passengers were Brazilian tourists flying to Barcelos to practice sport fishing on the Río Negro. The airplane departed Manaus-Eduardo Gomes Airport on this charter flight approximately two hours prior to the accident.

In unknown circumstances, it went out of control and crashed in a pasture located near the village of Las Velas, bursting into flames. The aircraft was destroyed by impact forces and a post crash fire and both occupants were killed.

Out of control, it crashed in a pasture located in the State of Zulia, bursting into flames. The crew fate remains unknown. The registration (c/n) must be confirmed, possible fake.

Shot down by the Venezuelan Authorities who suspected an illegal flight.

Shortly after takeoff from Concepción-Mariscal Francisco Solano López Airport, while climbing, the right engine caught fire. The pilot elected to return to the airport but was unable to and eventually decided to attempt an emergency landing. The twin engine airplane belly landed in an open field, slid for few dozen metres before coming to rest. All five occupants escaped with minor injuries and the airplane was damaged beyond repair.

Circumstances:
The airplane was on a schedule service (flight LA2213) from Lima to Juliaca. On board were 102 passengers and a crew of six. During the takeoff roll on runway 16L, after a course of 1,200 metres and at a speed of 131 knots, the aircraft collided with a fire truck that entered the active runway from the right, at taxiway Bravo. A team of two heavy fire trucks and one light support vehicle were engaged in a 'Response Time' exercise that started at 1510LT, one minute prior to the accident. The goal of this exercise was to test the capacity of the new fire station. The first fire truck entered the active runway while the airplane was accelerating for takeoff. Upon impact, the right main gear and the right engine were torn off. The airplane caught fire and slid for about 1,600 metres then veered to the right and came to rest, bursting into flames. Forty passengers were injured, among them four seriously. The airplane was heavily damaged by fire and written off. Among the personnel in the fire truck, two were killed and a third was seriously injured.
Probable cause:
It was determined that the crew of the fire truck entered the active runway 16L without prior permission from air traffic controller. This chain of events was caused by the lack of joint planning, poor coordination and the non-use of ICAO-standardized communication and phraseology.

While completing a turn on approach to runway 04, the airplane went out of control and crashed in a wooded area located about 3 km south of the airport, bursting into flames. The airplane was totally destroyed by impact forces and a post crash fire and all five occupants were killed. Puerto Ayacucho Airport is named Cacique Aramare but the military side is named José Antonio Páez.
