Circumstances:
Six technicians were attempting to start engines after a 60 hourly maintenance. The aircraft was not refueled after arriving some 36 hours previously on a flight to collect supplies. It had been standing out in the open in heat for some 16 hours with a possible built up of fuel fumes from leaky tanks or lines. The aircraft suffered an explosion from an electrical short, within the fuselage, causing a fire that consumed the aircraft. All six crew were injured and one of them died from his injuries few hours later. The aircraft was destroyed by fire.

The seaplane christened 'Bermuda Clipper' was fully loaded with passengers and crew, ready for departure from Manaus. A fire developed in the carburetor of the No. 1 engine. The pilot directed the flight mechanic to pull the fire extinguisher handle. The mechanic pulled the fuel dump handle by mistake. Flaming gas fell from the carburetor onto the gas on the water, igniting it, and causing the loss of the plane.

Circumstances:
The twin engine airplane was engaged in a local instructional flight at RCAF Mountain View, Ontario. While completing landing circuits, it caught fire in the air. During the approach, the pilot noticed smoke coming from the cabin, and after landing taxied off the runway, shut off the engines and investigated for the cause of the smoke. Flames broke out anew which could not be extinguished. There were no injuries among the crew but the airplane was totally destroyed by fire.

The airplane departed RAF Scampton at 2030LT on August 24 on an operation to Wesel. Following an uneventful mission, the crew returned to base where he landed at 0225LT. A few minutes later, an explosion occurred and the Hampden burst into flames. In addition to the crew, four members of the ground staff died in the explosion, caused, it is believed, by a bomb falling from the airplane as the bomb-doors were opened for an inspection of the bay.
