Following an uneventful flight from Habbaniya, the crew completed the approach and landing at Baghdad Airport and vacated the runway to join the apron. While on a taxiway, the undercarriage were mistakenly retracted by a crew member and the airplane sank on its belly. There were no injuries but the aircraft was damaged beyond repair.
Probable cause:
Error on part of a crew member who mistakenly retracted the undercarriage during taxi.

Circumstances:
The crew was performing a cargo flight on behalf of the 242th OCU and the airplane was carrying a load of various goods. En route, the crew informed ATC about the failure of the right engine that caught fire and requested permission to divert to the nearest airport. The crew was then vectored to RAF Dishforth and started the descent. Few minutes later, while still at an altitude of few hundred meters, the right engine detached. Out of control, the airplane nosed down and plunged into the earth, crashing in a field located 5 miles from Dishforth airbase. A crew who was able to bail out just before impact was seriously injured while three other occupants were killed.
Probable cause:
It was determined that a leak occurred on a hydraulic hose, causing the fluid to flow into the engine nacelle. While contact high temperature components, the fluid caught fire and later, the engine exploded and detached.

The crew was engaged in a supply mission over the Perak province. After dropping a load of various goods, the crew encountered technical problems with the left propeller that was overspeeding. The propeller was feathered when the aircraft lost height, struck trees and crashed in a dense jungle. Three crew members were killed while four others were injured. They walked away for four days before reaching a village.
Probable cause:
Left propeller oversped in flight.

Few minutes after takeoff from Croydon, while on a local training flight, the crew encountered poor weather conditions. Apparently, the pilot reduced his altitude in an attempt to establish a visual contact with the ground when the airplane clip trees and crashed in a wooded area located in Titsey Hill, about 7 miles southeast of Croydon Airport. Two crew members were rescued while two others were killed, among them S/Ldr Archibald Reginald Gerald Jackson.
