Circumstances:
The pilot was in charge to convoy the airplane from Cambridge to RAF Kinloss for maintenance. While cruising in poor weather conditions over Scotland, the right engine failed. Unable to maintain the prescribed altitude, the pilot attempted an emergency landing on the frozen Loch Laidon and the airplane came to rest near the shore. The pilot found help and was later transferred to his base while the airplane was damaged beyond repair.

The crew was completing a local training sortie at RAF Chivenor. While circling in the vicinity of the airbase at low altitude, the airplane impacted the water surface and crashed in the River Taw. Two crew members were killed and a third was injured.

Probable cause:
It was determined that the primary cause of the crash was the aircraft hitting the water of the River Taw, causing the port propeller to come off. A secondary cause was the stalling of the port wing during an attempt to carry out a forced landing due to:
a) the pilot attempted to reach the aerodrome with insufficient power or height to do so and
b) the use of flaps in the final stages of the approach.

The crew departed RAF Feltwell for an engine check. En route, the starboard engine failed, followed soon afterwards by the port engine. The crew attempted an emergency landing when the airplane stalled and crashed in a field at Lime Kiln Farm near Brandon, bursting into flames. Two crew members were killed and three others were injured. Few hours later, one of the survivors died from injuries sustained.
