Circumstances:
Owned by Skaidens (Skydance), the twin engine aircraft was engaged on local skydiving mission and operated on behalf of the Ukrainska Shkola Pilotov (Ukrainian Pilots' School). On board were 20 skydivers and two pilots. After take off from Borodyanka Aerodrome, the crew realized that weather conditions deteriorated and that a thunderstorm was approaching the airfield. The crew decided to cancel the mission and to return to the airport. On final approach, the aircraft encountered downdrafts and microburst. It lost height and crashed in a field some 900 meters short of runway threshold. Five skydivers were killed while 17 other occupants were injured, some seriously.
Probable cause:
According to the findings of the commission of inquiry, the most likely cause of the crash was the impact of the aircraft in a low-altitude wind due to strong downward air flow (micro-burst) during the landing of the aircraft in thunderstorms due to coincidence of the following negative factors.
- Failure of the crew to perform a go around or divert to the alternate aerodrome;
- A rapid increase in the speed of movement of the thunderstorm in the direction of the Borodyanka airfield area;
- Lack of training on the simulator in the conditions of wind shear, lack of experience in the crew on approach to landing in the conditions of wind shear, in particular micro-burst;
- Insufficient aeronautical equipment (lack of meteorological radar on the plane and airfield);
- Lack of information for the crew about the forecasted and actual meteorological conditions at the landing aerodrome, warnings about the forecasted / available wind shift at Borodyanka aerodrome;
- The crew was not sufficiently informed about the flight conditions due to insufficient lighting of the cockpit and failure of the instrumentation of the aircraft due to a power failure during approach in thunderstorm conditions;
- Lack of sufficient experience of the crew to perform activities and landings in conditions when the landing weight exceeded the maximum allowable, due to the presence of skydivers on board the aircraft;
- Motivation of the crew to perform the landing approach on the first attempt, due to insufficient information about the storm at the aerodrome. The information on wind increase and its direction (provided to the pilot) was perceived by the crew as possible conditions for landing because their parameters did not exceed the limits allowed by the AOM of the aircraft;
- Overloading of the aircraft, motivation of the decision of the captain to perform landing at the aerodrome of departure (Borodyanka) due to the presence of unregistered passengers on board, due to improper organization of boarding of skydivers at the aerodrome Borodyanka;
- Insufficient organization of flights at Borodyanka airfield in terms of meteorological support;
- Insufficient (weak) regulatory, regulatory, legislative framework for parachuting.

The crew was engaged in a spraying mission in the region of Knyazhichi, Sumy Oblast, Ukraine. Following 15 successful sorties that day, the crew was ready for the 16th and started the takeoff procedure from the third of the runway which is 768 metres long. After a course of 577 metres, the single engine aircraft failed to get airborne and overran. The crew decided to abort but this was too late. The aircraft sank in muddy field and rolled for about 56 metres before coming to rest upside down. Both occupants escaped uninjured while the aircraft was damaged beyond repair.
Probable cause:
The following factors were identified:
- The crew did not use the entire runway length for the takeoff procedure (577 metres instead of the 768 metres available),
- The decision of the crew to reject takeoff was taken too late,
- The wind direction changed during the takeoff procedure and became a tailwind component.

While manoeuvring at low height, the single engine aircraft hit tree tops, stalled and crashed, bursting into flames. The aircraft was destroyed by impact forces and a post impact fire and the pilot was killed.

The crew took off from a road located in Onikeyevo, Kirovograd oblast. After liftoff, the engine failed. The aircraft stalled and crashed near a farm, bursting into flames. Both pilots and the passenger, the owner of the farm, escaped uninjured while the aircraft was destroyed by a post crash fire. It appears that the flight was illegal as the CofA expired 26 April 2008.

The twin engine aircraft departed Hradec Králové on an exec flight to Kiev with four passengers and one pilot on board. On final approach to Kiev-Zhuliany Airport, the pilot encountered poor visibility (below minimums) and descended too fast and too low. On short final, at a speed of 220 knots, the aircraft impacted ground and crashed in a wasteland located 2,590 metres short of runway 08 and 48 metres to the right of its extended centerline. The aircraft was destroyed upon impact and all five occupants were killed, among them the CEO of the Czech company Minib that bought the aircraft last 28 November. According to Czech Authorities, the aircraft was to be transferred to the Czech Republic Registry within January 2008. At the time of the accident, the horizontal visibility was 900 metres and the vertical visibility was 100 feet due to low clouds.
Probable cause:
It was determined that the aircraft impacted ground after the pilot continued the approach in below minima weather conditions.
The following findings were identified:
- The pilot continued the descent below MDA until the aircraft impacted ground,
- Few seconds prior to the accident, ATC instructed twice the pilot to initiate a go-around procedure, but this was too late,
- The pilot was approaching the airport at an excessive speed and below the glide,
- Poor visibility due to low clouds (below minimums).
