The aircraft was engaged in a cargo flight from Voronezh to Susuman with intermediate stops in Kurgan, Krasnoyarsk and Lensk, carrying four passengers, five crew members and a load of 3 tons of sausages and cheese. After departure from Kurgan Airport, a fuel pump failed and the crew decided to divert to Novosibirsk-Tolmachevo Airport where repairs were successfully conducted. En route to Susuman, while cruising at an altitude of 5,700 metres, the crew got disoriented and the radio contact were difficult. As the fuel reserves were low, the captain decided to attempt an emergency landing 75 km southeast of Zyrianka Airport. With flaps down at 38° and at a speed of 177 km/h, the aircraft crash landed and slid for 488 metres before coming to rest. All nine occupants escaped with minor injuries and the aircraft was damaged beyond repair.
Probable cause:
The accident was the result of an emergency landing made necessary after the crew lost his orientation following successive errors during pre-flight and en route. The following contributing factors were reported:
- Poor flight planning and organization,
- The captain took the unjustified decision to change heading,
- ATC in charge of the Okhotsk region authorized the crew to change heading without knowing the exact position of the aircraft,
- Upon departure, the total weight of the aircraft 5,230 kilos above MTOW,
- Radio communication issues.

Circumstances:
The aircraft was engaged in a cargo flight from Chersky to Zyrianka, carrying two loadmasters, five crew members and a load consisting of eight wooden copper coils. During the takeoff roll, at a speed of 200 km/h, the pilot-in-command started the rotation (the rotation speed was calculated at 215 km/h). The airplane rotated and continued to climb until the altitude of 150 meters when it entered stall conditions. It turn to the right and crashed in a huge explosion on an hotel located near the airport. The aircraft and the hotel were totally destroyed by a post crash fire. All seven occupants were killed while no one on the ground was injured.
Probable cause:
It was determined that the stall and subsequent loss of control during initial climb was caused by the cargo shifting. Investigations revealed that the wooden copper coils were not properly installed and secured in the cabin but only fixed with the onboard winch cable. At liftoff, the wooden copper coils moved to the rear of the cabin, causing the CofG to be out of the enveloppe and the aircraft to be uncontrollable. Loadmasters failed to install and secure the wooden copper coils according to the published procedures and failed to use appropriate equipment such as nets or belts. The copilot exercised an unsatisfactory supervision of the loading while the captain decided to takeoff despite the load was improperly secured.

On a flight from Zyryanka to Seymchan in support of the 121st geodetic expedition, the crew encountered below-minima weather conditions due to low clouds. The crew failed to return and deviated from the prescribed flight path by 25 km and descended in clouds over mountainous terrain. In such conditions, the aircraft crashed at a height of some 1,200 metres into the steep slope of Mount Khapchagai (1,554 metres high) located 61 km northwest of Seymchan. One of the 2 pilots and 4 of the 6 passengers were killed and all survivors were seriously injured. The wreckage and the survivors were found a day later, on 23SEP68.
Probable cause:
Failure of the crew who decided to continue the flight into below-minima weather conditions and failed to return to its departure point.

The crew was performing a cargo flight from Pevek to Magadan with intermediate stops at Zyrianka and Seymchan, carrying a load of reindeer skins for a total weight of 800 kilos. Due to strong winds, the airplane deviated from the prescribed flight plan and nor the crew nor ATC were able to note this. While approaching a mountainous area in poor weather conditions, the crew encountered downdrafts. The airplane lost altitude and descended from 2,400 to 1,820 meters in few seconds before crashing on the slope of Mt Tuonnyakh. The wreckage was found four days later some 53 km northwest of Seymchan and all four crew members were killed in the accident.
Probable cause:
The crew failed to follow navigation rules and failed to maintain regular communication contacts with ATC who also failed to assist the crew properly. Information transmitted to the crew and related to wind component did not reflect the truth, which was considered as a contributing factor.

The aircraft was performing a cargo flight from Magadan to Pevek with intermediate stops at Seymshan, Zyrianka, Nizhni Kresty, and Bilibino, carrying one passenger, a crew of five and two tons of apples. The takeoff was started by night and in snow falls from a contaminated runway. After a long takeoff course, the pilot-in-command completed the rotation at low speed. After liftoff, the airplane reached a high angle of attack, stalled and struck the runway surface several times. It continued for few hundred yards then banked left and eventually crashed in flames in a snowy field located 440 meters past the runway end. The passenger was slightly injured while all five crew members were killed. The aircraft was destroyed.
Probable cause:
The crew took the decision to takeoff from a contaminated runway that has not been properly cleaned prior to the accident. In such conditions, all safety conditions were not met and the aircraft performances were reduced. For undetermined reason, the crew decided to continue the takeoff run despite the course was too long and failed to take the right decision to abandon the departure procedure.

Shortly after takeoff from Zyrianka Airport, while climbing to a height of about 20 meters, the left engine encountered problems and the crew tried to feather the propeller, without success. In such conditions, the pilot-in-command decided to attempt an emergency landing. At a speed of 140 km/h, the airplane hit trees and crashed in a wooded area located 600 meters past the runway end. The airplane was damaged beyond repair while all 16 occupants were rescued.
Probable cause:
When the undercarriage were raised after liftoff, the left engine suffered technical issues and the crew was forced to feather the propeller, without success. The propeller was then windmilling, causing drag. In such conditions, the aircraft lost height and the pilot elected to make an emergency landing. It was reported that the board mechanic was unable to response immediately to the engine issue because he was focused on instruments check and especially the speed indicator, which was considered as a contributory factor.

The crew was performing a cargo flight from Pevek to Zyrianka, carrying a crankshaft and others spare parts for an LM-3 locomotive for a total weight of 1,000 kilos. The airplane left Pevek at 0410LT bound for Zyrianka but upon arrival, the crew was informed by ground that the airport was closed to traffic and has to divert to Seymchan Airport. The pilot modified his route to Seymchan but did not receive any information about weather conditions at destination. After continuing at an altitude of 3,300 meters, the crew encountered poor weather upon arrival in Seymchan and was unable to locate the airport. After an unsuccessful attempt to land, the crew continued to the north and was eventually forced to make an emergency landing as the aircraft was short of fuel. The airplane crashed in a rocky canyon located 72 km northwest of Seymchan and was destroyed upon impact. All five crew members were killed.
Probable cause:
The accident was caused by the combination of the following factors:
- The visibility was less than 100 meters at the time of the accident due to snow falls,
- The copilot was inexperienced,
- Wrong decision in diverting the aircraft to Seymchan without knowing the weather conditions at destination (uncertain situation),
- The total rest time of the crew was insufficient,
- Lack of assistance by ground services upon arrival in Seymchan,
- Wrong decision on part of the Zyrianka Airport Authorities which decided to close the airfield to all trafic because the runway was flooded. Investigations revealed that this was not the case and this decision was taken in rush. When the aircraft approached Zyrianka, the airfield was practicable with a 1,000 meters long runway.

While approaching Srednekolymsk, weather conditions worsened and the radio contact with ATC was lost. The approach was abandoned and the pilot was forced to make a go around as he was unable to locate the airport. A second and a third attempt to land were abandoned as well. While completing a last turn during a fourth approach, the pilot lost control of the aircraft that stalled and crashed in the Kolyma River, about 400 meters short of the runway threshold. Three passengers were seriously injured while 23 other occupants were killed. It appears that the aircraft' speed was too low during the last turn.

The crew was engaged in a rescue mission from Susuman to Zyrianka on behalf of the Ministry of Interior (MVD – Ministerstvo Vnutrennikh Del). Shortly after takeoff, the left engine failed and the pilot increased power on the right engine that failed as well. The crew elected to return to Susuman when the aircraft went out of control and crashed in a snowy field located in Berelyokh, about 10 km from Susuman. The aircraft was destroyed and all 7 occupants were killed. Some repairs were recently performed on the right engine and it appears the aircraft was not tested before the flight.
Probable cause:
Double engine failure.

Out of control, the aircraft veered to the right and eventually crashed in a snow covered field. Six occupants were slightly injured while a pilot was killed. The aircraft was destroyed.
Probable cause:
At the time of the accident, the aircraft was carrying a load of 1,125 kg of various goods plus 1,400 liters of fuel. It was determined that the total weight of the aircraft was well above the MTOW. Also, the crew did not deice the airplane prior to departure and failed to clear the wings from snow, which was considered as a contributory factor.
