Circumstances:
The single engine aircraft departed Lensk at 0813LT on a flight to Bratsk. While cruising at 4,200 metres over the cloud layer, the engine failed. The crew elected to divert to Kirensk Airport but was unable to maintain a safe altitude. Eventually, the captain attempted an emergency landing when the aircraft impacted trees and crashed in a wooded area located 37 km west of Kirensk. All nine occupants were injured and the aircraft was damaged beyond repair.
Probable cause:
Engine failure due to the damage of the bearings of the planetary gear from the first stage of the compressor, leading to vibration and destruction of the turbine. It is possible the damage to the bearings was caused by the presence of aluminium or silicon oxide. However, it was not possible to determinate the source of this contamination.

Circumstances:
The crew initiated the descent to Irkutsk-Intl Airport by night and was cleared to 3,000 meters then 2,100 and 1,800 meters. The crew failed to realize his altitude was insufficient when the aircraft struck trees and crashed on the top of a hill (700 meters high) located 21 km southeast of the airport. The aircraft was destroyed and all four occupants were killed.
Probable cause:
It was determined that the crew descended below the minimum descent altitude following a wrong altimeter setting. At the time of the accident, the crew was very tired due to a duty time over 15 hours and 26 minutes.

The aircraft was engaged in a cargo flight from Ust-Kut to Kirensk, carrying a crew of three, two technicians of the company and two people in charge of the 1,200 kg of cargo. The crew failed to prepare the flight properly, expedited taxi and departure. The airplane took off from runway 30 at a speed of 155 km/h and with an angle of attack of 15°. After rotation, while in initial climb at a height of 120 metres at a speed of 210 km/h, the pilot raised the flaps when the airplane lost height and descended until it struck trees and crashed in flames 1,500 metres past the runway end. The copilot was seriously injured while all six other occupants were killed. Few hours later, the copilot died from his injuries.
Probable cause:
It was determined that the crew precipitated the takeoff procedure and failed to follow the pre-departure checklist. The following contributing factors were reported:
- Poor flight preparation,
- The cabin was not heated prior to departure,
- The left altimeter was erroneously set with a value 10 mm Hg higher than prescribed (wrong altimeter setting),
- Stabilizers were set at -6° instead of the required -3,6° angle,
- The rotation was completed prematurely,
- During initial climb, flaps were raised at a speed of 210-220 km/h, 20 km/h below the speed mentioned in the Flight Manual,
- Lack of visibility,
- Lack of ground visual references.

The crew was returning to Irkutsk following a cargo flight to Mirnyy. After takeoff from Kirensk Airport, while climbing to a height of some 500 meters, the airplane banked left, dove into the ground and crashed onto a wooden hut located 750 meters south of the airfield. The airplane was totally destroyed and all six crew members were killed.
Probable cause:
It was determined that both left engine lost power shortly after takeoff and progressively failed during climb. The crew elected to feather the propeller but without success. The airplane then banked to the left and crashed at an angle of 70°. Investigations were unable to determine the exact cause of the engine failure but the assumption of an electrical short-circuit or an excessive accumulation of ice in the fuel filter was not ruled out.
