The wreckage was found 735 meters to the left of the extended centerline. Both pilots and four passengers were killed while eight other occupants were injured.
Probable cause:
The exact cause of the accident could not be determined with certainty. However, the following findings were reported:
- It is believed that the crew got distracted during the last portion of the final approach,
- Lack of visibility due to the night,
- The aircraft was not properly aligned on the approach path,
- The crew failed to follow the published approach procedures,
- The crew failed to pass over the outer marker, causing the aircraft to follow an erratic approach trajectory,
- The crew switched their seats as the captain was seating in the right front seat and the copilot in the left front seat, which is prohibited in such operations,
- The radio altimeter alarm failed to sound due to wrong settings,
- The aircraft banked left to an angle of 48° and nosed down to an angle of 24° just before final impact.

Circumstances:
The crew initiated the descent to Irkutsk-Intl Airport by night and was cleared to 3,000 meters then 2,100 and 1,800 meters. The crew failed to realize his altitude was insufficient when the aircraft struck trees and crashed on the top of a hill (700 meters high) located 21 km southeast of the airport. The aircraft was destroyed and all four occupants were killed.
Probable cause:
It was determined that the crew descended below the minimum descent altitude following a wrong altimeter setting. At the time of the accident, the crew was very tired due to a duty time over 15 hours and 26 minutes.

Circumstances:
After being cleared, the crew initiated the descent from 3,000 meters to 2,700 meters then continued when, at an altitude of 2,226 meters, the twin engine aircraft struck the slope of a mountain located 74 km from Bagdarin Airport. The aircraft disintegrated on impact and all 17 occupants were killed. At the time of the accident, the visibility was poor due to low clouds and snow falls.
Probable cause:
It was determined that the crew made a wrong flight time calculation and misevaluated the climbing time, causing a discrepancy of 11 minutes. Due to radio communications problems and also unstable radiogoniometric systems, ATC was unable to establish the exact position of the aircraft. The crew started the approach prematurely, causing the aircraft to descent below the minimum safe altitude. Due to lack of visibility, the crew was unable to see and avoid the mountain.

Circumstances:
During the takeoff roll on the gravel Airstrip at Ust-Maya Airstrip, after passing V1, the left turbocompressor lost power then the engine stopped, 1,8 second before V2 speed. Power on the right engine was increased till 96% and the crew continued the takeoff procedure. Liftoff was completed at a speed of 160 km/h then the aircraft climbed to a height of 50 meters and entered a left turn. Slowly, the altitude dropped and the aircraft eventually crashed in the Aldan River about 270 meters offshore. The wreckage sank and was found 31 hours later. All 14 occupants were killed.
Probable cause:
It was determined that the failure of the left engine was the consequence of errors on part of the crew who failed to follow the pre-takeoff engine checklist. The aircraft's speed at takeoff was insufficient and the engine failure occurred at a critical stage of the takeoff procedure, causing a loss of controllability after liftoff. During climbout, the speed decreased because the crew did not raise the landing gear due to high workload caused by an emergency situation.
