The floatplane was departing Churan on a service to Irkutsk, carrying seven passengers and a crew of four. Shortly after takeoff from the hydrobase of Churan, while climbing to a height of 10-12 metres, the floatplane entered a high angle of attack, pitched up then the left wing stalled and the aircraft crashed into the Lena River. While all four crew members were able to evacuate the cockpit and to swim to shore, all seven occupants drowned after the airplane sank.
Probable cause:
Failure of the pilot to position correctly the stabilizers prior to takeoff and his failure to reject the takeoff procedure while the aircraft was unstable.

Circumstances:
The airplane departed Kulob Airport at 1100LT on a schedule service to Stalinabad, carrying 11 passengers (nine adults and two children), two pilots and a total of 265 kg of luggage. Ten minutes after takeoff, while cruising at an altitude of 940 metres above a mountainous area, one of the engine failed. The crew attempted to make an emergency landing but there were no suitable terrain. The airplane lost height and impacted the slope of a rocky mountain (slope of 60°) located 18 km northwest of Kulob. The wreckage was found less than an hour later some 15 metres below the summit. The airplane disintegrated on impact and all 13 occupants were killed.
Probable cause:
Due to the degree of destruction of the airplane and the lack of witnesses, the exact circumstances of the accident could not be determined. Nevertheless, one of the engine failed in flight, most probably due to the failure of a fuel supply and/or the carburetor where irregular and abnormal wears were found, because of a poor polish during the last repair session.

Circumstances:
The pilot, sole on board, was engaged in a crop spraying mission on beets field located near the village of Budenovka. Shortly after takeoff (last flight as the operation started at 0500LT), at a height of 20-25 metres, the pilot initiated a sharp turn to the right. The airplane rolled to an angle of 45° when the pilot reduced the engine power, causing the aircraft to stall and to crash. The airplane was destroyed and the pilot was killed.
Probable cause:
Poor piloting techniques on part of the pilot who initiated a sharp turn to the right at an insufficient height (below the minimum height of 50 meters) and reduced power too early.

Captain / Total hours on type:
13.00
Aircraft flight hours:
432
Circumstances:
The pilot was engaged in a local training flight at Minsk Airport. Following several circuits in the vicinity of the airfield, he initiated the approach. Not properly aligned with the runway, the pilot decided to make a go around. At a height of about 50 metres, the airplane continued for about 300 metres then lost height, rolled to the left and crashed. The pilot was seriously injured and died seven hours later.
Probable cause:
It was determined that the pilot failed to position the stabilizers in a correct position for landing (negative instead positive angle). He was not familiarized with the aircraft and his experience on type was insufficient with 13,8 hours of flight. It was also reported that the instructor released the pilot too early for a solo flight.
