The pilot was in charge to transfer the airplane from RAF Castle Bromwich when an engine failed in flight. He attempted an emergency landing when the aircraft crash landed in Hanney Fields north of Wantage. The pilot was uninjured and the airplane was written off.

Circumstances:
The airplane was engaged in a tour of Gabon to inspect various airfields, and departed Libreville on a flight to Douala with intermediate stops in Gomé, Lambaréné, Booué and Bitam. On board were four crew members and four passengers, among them Colonel Parant who wanted to visit the airfield of Bitam that was recently open to traffic. On final approach, the pilot failed to realize his altitude was too low when the undercarriage collided with a 40 cm high mound of earth located short of runway threshold. Upon impact, the airplane overturned and crashed upside down. Six occupants were slightly injured while Colonel Parant was seriously wounded. He died from injuries sustained on 15 March 1941.

Circumstances:
The airplane departed Ladd AAF in Fairbanks on a flight to Wright-Patterson AFB in Dayton with intermediate stops in Sacramento Air Depot and Lowry AFB in Denver. En route from Sacramento to Denver, the airplane impacted the slope of the Ragged Top Mountain located about 19 miles southwest of Lovelock. The aircraft was destroyed and all eight occupants were killed. The aircraft was equipped with the top secret Norden bombsight and sabotage was suspected as the cause of the crash, but was never proven.

The airplane departed Guangzhou (Canton) Airport on a flight to Haikou, Hainan Island, carrying six high ranking officers of the Imperial Japanese Navy performing an inspection of the Japanese troops in the occupied South China and three crew members. While flying about 100 km south of Guangzhou, the airplane was shot down by enemy fire and crashed in Huangyangshan, on the west bank of the Xijiang River. All nine occupants were killed, among them Admiral Mineo Ōsumi, ex Marine Minister.
