The airplane failed to return to base and was lost without trace. Both occupants were considered as deceased.

The floatplane crashed upon landing on Onega River off Gryaznaya. Two crew members were killed.

The airplane departed Moscow-Khodynka Airport on a flight to Kuybyshev, carrying four crew members and 20 passengers, in connection with the evacuation of GAZ n°1 to Kuybyshev. En route, the crew encountered poor weather conditions with low clouds, fog and freezing drizzle. Some 25 km east of Kuznetsk, the crew flew some circles over a forest located on a 340 metres high hill near Lake Beloye when the airplane crashed for unknown reasons near Golodyayevka (now Dubrovka). All 24 occupants were killed, among them the aircraft designer Vsevolod Tairov.

Shortly after takeoff from Shibenets Airfield, the airplane failed to gain sufficient height, stalled and crashed into a ravine. The airplane was destroyed and all nine occupants were injured.

The airplane departed Moscow central airport on a special flight to Cherepovets and Tikhvin, carrying 13 mechanics and four crew members. The purpose of the mission was to ferry mechanics to these two airports to proceed with repairs and maintenance on several grounded airplanes. On the leg from Cherepovets to Tikhvin, the airplane was shot down by the pilot of a JU.88. The pilot of the PS-84 attempted to divert to Podborovye Airfield for an emergency landing but on short final, the airplane went out of control and crashed 300 metres from the airfield. A passenger was seriously injured while 16 other occupants were killed.

The crew encountered an unexpected situation and was forced to make an emergency landing when the floatplane crashed into the sea off the coast of Murmansk oblast. All three crew members were killed.
