Circumstances:
Six minutes after takeoff from Karaganda Airport, while climbing, the crew reported to ATC that the left engine failed and obtained the permission to return. The pilot in command completed a circuit and started a descent. On final approach, while completing a last turn to the left at a height of 60 meters, the aircraft stalled and crashed in a huge explosion in a field located short of runway threshold. The aircraft was destroyed by impact forces and all 25 occupants were killed.
Probable cause:
Investigations were unable to determine the exact cause of the failure on the left engine. However, it was reported that the crew already encountered technical problems with the same engine a day prior to the accident but failed to report these problems to the ground maintenance.

Few minutes after its takeoff from Balkhash Airport, while climbing, the aircraft flew into clouds when control was lost. The aircraft banked right and lost altitude. Shortly later, the right wing detached and out of control, the aircraft dove into the ground and crashed in an isolated area located about 39 km northwest of Balkhash. The wreckage was spotted 10 days later, on October 10, and all 15 occupants were killed.
Probable cause:
It was determined that the pilot in command suffered a spatial disorientation while climbing into clouds. Also, the right wing detached presumably because of aerodynamic forces that exceeded its design.

Circumstances:
The crew was performing a ferry flight from Alma Ata to Kuybyshev with an intermediate stop in Semipalatinsk. En route, the pilot in command deviated from the prescribed flight path to the east in order to circumvent an area of poor weather and overflew high mountains. As he was unable to gain sufficient height, the three engine aircraft stalled and crashed at a height of some 2,700 meters into the steep wall of a gorge near a pass over the khrebet Dzhil-Dhuta mountain range (3,050 meters), about 15 km northwest of Koksu. All six occupants were killed.
Probable cause:
The decision of the crew to modify their route to the east was wrong. It was preferable to fly to the west to avoid the mountain range, especially in poor weather conditions. It was reported that the navigator was not aware of the presence of mountains above the altitude of 3,000 meters, which was considered as a contributory factor. Also, deficiencies were pointed out with flight planification and the operations of the company.

Circumstances:
Built last December 2, the aircraft was on its way from the Tashkent construction plant to Aktyubinsk via Shalkar to be delivered to the Soviet Air Force. While cruising vertical to the Mugodzhar mountain range, the crew encountered poor weather conditions with low clouds and icing. The aircraft lost height and crashed on the south slope of the Mt Menshy Brat (590 meters high). The wreckage was found two days later at an altitude of 520 meters and all eight occupants have been killed.
Probable cause:
The pilot was blamed to continue the flight in such poor weather conditions, and it would be better to return to his last departure point.

Five crew members and 21 passengers were killed while eight other occupants were seriously injured.

At Shalkar Airport, he was informed about the poor weather conditions en route to Aktyubinsk but decided to takeoff, again, without authorization. About an hour and 45 minutes into the flight, while flying in clouds, he lost control of the airplane that crashed in a prairie located 3 km from Alga Station, about 45 km southeast from Aktyubinsk. The aircraft was destroyed and both occupants were seriously injured.
Probable cause:
The decision of the pilot to takeoff without permission, knowing that weather conditions en route were unfavorable. It was also reported that the airplane was not equipped with any navigational instruments or any equipment that could allow the pilot to flight under IMC conditions.

The airplane entered clouds and the pilot decided to return so he initiated a turn to the right when the airplane impacted the slope of a mountain and crashed. The aircraft was destroyed and the pilot was killed.
Probable cause:
The following factors were identified:
- Lack of discipline on part of the pilot by failing to obey the order given to him upon departure,
- Violation of the Flight Manual on part of the pilot,
- The release of an inexperienced pilot on a charter flight in bad weather,
- The weather bulletin was valid for three hours and thus was not accurate anymore at the time of the accident.
