During the take off run, a landing gear failed. The twin engine aircraft went out of control, veered off runway and came to rest. While nobody was hurt, the aircraft was damaged beyond repair.
Probable cause:
Landing gear failure.

Circumstances:
The aircraft left Townsville at 0815LT bound for Port Moresby. En route, the crew encountered poor weather conditions with heavy rain falls. While cruising at the altitude of 3,000 feet above the Hinchinbrook Island, the aircraft hit the south slope of Mt Straloch located in the south part of the island. All 12 occupants were killed. The wreckage was eventually found by locals in 1943.

Circumstances:
En route from Townsville to Cooktown, the pilot was forced to ditch the aircraft into the Coral Sea for unknown reason. While the aircraft christened 'The Apple Cart' sunk and was lost, the crew took refuge in a dinghy and was rescued 18 hours later by the crew of a freighter.

The crew left Garbutt Airfield in Townsville at 2300LT on September 17 on a bombing mission over Rabaul. On the return leg, the weather conditions worsened and the visibility was poor. In a last radio message, the on board operator informed ground that the crew will abandon the aircraft that crashed near Weipa. Eight crew members were rescued by the crew of a Catalina on September 20. Unfortunately, the ninth occupant was killed as his parachute did not open.

The crew left Townsville on a flight to Port Moresby (7 Mile Aerodrome). About thirty minutes prior to the ETA, the crew encountered poor weather conditions and the visibility was low. The captain was able to gain height and continued above the cloud layer. Unfortunately, the aircraft christened 'Yankee Clipper' overflew Port Moresby and continued to the north. Due to fuel shortage, all eight occupants decided to abandon the aircraft and bailed out. While the aircraft crashed and was lost, six crew members were recovered (some of them eight days later) while two others were killed.

Circumstances:
On final approach to Innisfail Airport, the pilot saw a tractor on the runway and initiated a go around procedure. Doing so, the airplane names 'Cairns' stalled and crashed short of runway, bursting into flames. The pilot and four passengers were killed while four other occupants were seriously injured. The aircraft was totally wrecked.

Probable cause:
Investigation stated the aircraft was correctly loaded, 180lb below Max All Up Weight, had flown 4,680 hours of which 3,620 hrs were in Australia over previous 23 months. Pilot Goodson had been with the airline only 3 days and had gained his Dragon endorsement flying a service to Townsville return not fully loaded and without landings en route. Goodson had 2,000 hours experience and had been an instructor with Rockhampton Aero Club, later CFI of Kingsford Smith Aviation Services, Sydney but had never flown a Dragon. He allowed the aircraft to stall during a discontinued approach because of a tractor on the runway.
