Circumstances:
The four engine aircraft was transferred from Kisangani to Mykolaiv, Ukraine, for maintenance purposes. Two enroute stops were scheduled in Entebbe and Luxor. Ar 0417LT, the crew was cleared for takeoff. The aircraft started rolling on runway 02 for takeoff but it kept rolling on it till it reached its end. The aircraft continued rolling in the sand drifting from the runway centerline to the right, crossing a service road and eventually crashing into a military zone located 500 metres from the runway end. The aircraft crashed into one of the buildings inside the military unit causing complete destruction of the building. The right wing also crashed into another building putting the aircraft on fire and forcing the aircraft to turn right. The aircraft kept moving forward the military unit fence (next to Tiba-Luxor road) and stopped there, crashing in its way into oil barrel. The accident resulted in the total destruction and fire of the aircraft and the fatality of its crew but no further injuries.
Probable cause:
The following factors were identified:
- Lack of available thrust which corresponded to the power of only two engines running during take off run,
- Lack of authorized maintenance of the aircraft,
- The uncoordinated crew actions in the critical situation (both pilots had previously flown as captains). The lack of coordination is confirmed by the braking trails left by the main landing gear at the end of RWY -02, based on FCOM instructions, and in case of rejected takeoff attempt, the crew had to set the throttles to idle, lower the nose gear to reach the runway and then use the propellers for braking (releasing them from the stops). Also, to reduce the roll and to maintain the direction they were to use the nose wheel steering and wheel brakes as well as emergency braking if needed. Actually the crew did neither of the above procedures, except using the main wheel brakes.

Circumstances:
The aircraft was engaged in a cargo flight from Ras al-Khaimah to Mykolaiv, carrying eight crew members and a load of various goods among them few tons of water melons. Following a night takeoff from runway 34 at a speed of 235 km/h, the aircraft reached the altitude of 150 metres at a speed of 365 km/h then continued to 160 metres at a speed of 400 km/h. The crew raised the flaps in five steps when the aircraft entered an uncontrolled descent with a rate of 12 metres per second until it impacted the sea at a speed of 485 km/h about 16 km from the airport, some 800 metres offshore. The aircraft disintegrated on impact and all eight occupants were killed.
Probable cause:
It was determined that the aircraft was overloaded by 15-25 tons regarding the actual flight conditions and 13-23 tons regarding the MTOW as published in the manuals. At the time of the accident, the OAT was +37° C, reducing climb and flight performances.

Circumstances:
During the takeoff roll at Kacha Airbase, the pilot started to rotate but the aircraft failed to respond. The captain decided to abandon the takeoff procedure and initiated an emergency braking maneuver. Unable to stop within the remaining distance, the aircraft veered off runway to the left and rolled in a rough terrain where it came to rest, bursting into flames. All occupants escaped uninjured while the aircraft was destroyed by fire.
Probable cause:
It was determined that the crew failed to prepare the flight according to published procedures and failed to follow the pre-takeoff checklist as the rudder had not been unlocked prior to takeoff. It was also reported that the crew took the decision to abort five seconds too late.

Circumstances:
Enroute, one of the crew member opened the right cockpit window and threw away his cigarette that went into the air inlet of a ventilation system. A fire spread quickly and as the crew was unable to extinguish it, the captain decided to reduce his altitude and to attempt an emergency landing. The airplane belly landed in a snow covered field located near Lipovets, slid for few hundred meters and came to rest in flames on the iced Rosava River. Two crew members and one passenger were killed while ten other occupants were injured. The aircraft was written off.
Probable cause:
The aircraft has been subject to technical modifications on part of the Aeroflot technicians few days prior to the accident and converted to a cargo version capable to carry a maximum of 9 passengers. It was reported that several mistakes were done during this conversion, especially concerning the ventilation system air inlet that was installed in a wrong position, just behind the cockpit window instead of lower or higher. Investigations revealed that the fire spread quickly because the insulating material used failed to respond to the requirements and was unimpregnated wadding. The aircraft accumulated 73 flying hours since this conversion.

Circumstances:
The airplane departed Bucharest on a flight to Mykolaiv. Approaching Odessa, the crew encountered poor weather conditions. Control was lost and the airplane crashed into the sea. All nine occupants were killed. In 2009, the wreckage was found by a depth of 23 metres off Odessa.
