Circumstances:
The three engine aircraft departed Abadan for a night flight to Mashhad. Due to poor weather conditions at destination, the crew diverted to Isfahan Airport. The aircraft departed Isfahan Airport at 0535LT bound to Mashhad. While on an ILS approach in thick fog, the aircraft was in a nose high attitude when the base of the empennage struck the runway surface and separated. On impact, the undercarriage were torn off. Out of control, the aircraft slid for few dozen metres, veered off runway and came to rest with both wings partially torn off, bursting into flames. At least 46 occupants were injured while the aircraft was partially destroyed by fire. Vertical visibility was 200 feet at the time of the accident due to fog.
Probable cause:
The following findings were reported:
- The visibility was below minimums,
- The crew continued the approach despite the aircraft attitude was incorrect,
- The crew failed to initiate a go-around procedure.

While taxied by an unauthorized mechanic, the twin engine airplane went out of control and collided with a parked Fokker F27 Friendship registered PH-IOT and also operated by the Iraanse Aardolie Exploratie en Productie Mij (IAEPM). The mechanic was unhurt, the Dove was damaged beyond repair and the F27 was repaired.

The seaplane was transferred from Biak to The Netherlands when the crew encountered technical difficulties en route so a stop was made at Abadan. After two weeks maintenance, permission was given to the crew to continue to Europe but shortly after takeoff, an oil leak on engine number one forced the crew to return to Abadan Airport. On approach, the airplane went out of control and crashed in a huge explosion few km from the airfield, killing all ten crew members.
Probable cause:
A technical failure occurred on the engine number one whose thrust reversal system was accidentally triggered for reasons that investigations were unable to determine.

About eight minutes after takeoff from Abadan Airport, while on a short flight to Kuwait City, the crew was flying at an altitude of 500 feet when power was lost on engine number three and four. The propeller on engine four was feathered and the crew contacted ATC for to return to Abadan for a safe landing. As the airplane lost height, the captain eventually decided to attempt an emergency landing in a desert area located in Al Seeba, at the border between Iraq and Iran. The aircraft belly landed and was damaged beyond repair. All 13 occupants were evacuated safely.
Probable cause:
Mismanagement of the fuel pumps on part of the flying crew after departure from Abadan Airport. There was less than 100 gallons of fuel on the right tank at the time of the accident, which was not sufficient and thus considered as a contributing factor.

While descending to Tehran-Mehrabad Airport in limited visibility due to heavy fog, the crew was instructed by ATC to divert to Isfahan Airport. The pilot-in-command abandoned the approach and modified his route, while confirming to ATC that his fuel reserves were insufficient to cover the distance. Few minutes later, the crew informed ATC that he was short of fuel and elected to make an emergency landing when the aircraft crashed in a field located about 7 km from Mehrabad Airport. Rescuers arrived on scene two hours later and were able to evacuate two passengers who were seriously injured, all 27 other occupants have been killed.
Probable cause:
The accident was caused by a fuel exhaustion. The lack of visibility due to foggy conditions was considered as a contributory factor, as well as the fact that the crew miscalculated the necessary fuel quantity to possibly fly to an alternate airport.
