Circumstances:
The pilot, sole on board, departed Ozeryany on a private flight to Gardychyvka. En route, weather conditions deteriorated with low clouds and fog. The pilot decided to continue and reduced his altitude to maintain a visual contact with the ground when the aircraft struck the ground and crashed in a snow covered field. The pilot was seriously injured and the aircraft was destroyed.
Probable cause:
The following findings were identified:
- The documentation was incomplete as there was no insurance certificat on board, no flying permit and no CofA,
- The flight was completed with one pilot only, without any copilot,
- The pilot failed to inform ATC and the Ukraine Army about the flight and did not receive any permission to perform the flight,
- When weather conditions worsened and the visibility dropped below minimums, the pilot failed to return or to divert to the alternate airport, continued and reduced his altitude until the aircraft impacted ground,
- Weather information obtain by the pilot prior to the flight did not correspond to the actual and forecaster weather at the time of departure,
- The operation of the aircraft with the ASh-62IR engine (s/n K1641652 was prohibited.

Circumstances:
The single engine aircraft was engaged in a local skydiving mission, carrying one pilot and 9 skydivers. Shortly after take off from runway 31, while climbing to a height of about 80 meters, the engine suffered a power loss. The pilot initiated a left turn to reach the airport when the aircraft rolled to an angle of 60° then stalled and crashed in a snow covered field near a forest, bursting into flames. The aircraft was destroyed by a post impact fire. Nine occupants escaped injured and a passenger was killed.
Probable cause:
Most probably the accident with and An-2 RA-33589 aircraft was caused by aircraft beyond stall angle of attack entry during dynamic turn for emergency landing with reverse runway heading due to engine failure after take-off at low-altitude followed by shaking and its power loss resulted in aircraft ground impact with bank angle more than 60°. Engine failure was caused by high-temperature damage and destruction of cylinder-piston group details due to it inadequate technical operation and using motor gasoline in aircraft fuel flow system which quality didn't comply with requirements for preferred AI-95 gasoline in accordance with aeronautical equipment flight service evaluation program, Order of Ministry of Transport of the Russian Federation № НА-131-р from April 11, 2001 and engineering solution № АБ-1236-2003 from May 22, 2003. Most probably the contributing factor was partial skill loss of An-2 aircraft control by aircraft pilot in command (PIC). Flight operation was performed by PIC, meteorological minimas and which authorizations written in Private-Pilot License and the fact of medical flight-expert commission procedure were not documented. PIC's decision to perform straight-ahead landing most probably didn't allow to reduce the severity of accident consequences.

Circumstances:
The crew was engaged in a patrol flight (fire survey) from Pobeda when the engine failed in flight due to low oil pressure. The crew attempted an emergency landing on the road from Komsomolsk-on-Amur to Elban, near Padali (58 km from Komsomolsk). The aircraft suffered damage to the left main gear and the left lower wing and was damaged beyond repair. All three occupants escaped uninjured.
Probable cause:
Engine failure in flight following a loss of oil pressure.

Circumstances:
In the evening of 11JUN2012, a group of 13 people departed Serov Airport at 2200LT on an illegal trip with an unknown destination, maybe for fishing, sauna or any other party. The aircraft failed to return and was lost without trace. Local Authorities performed search and rescue on more than 275,000 square km until 23JUL2012 without success. It seems that all occupants were intoxicated when they took the airplane, among them the Chief of Serov City Police, three of his deputies, an airport guard and several others. On Saturday 05MAY2013, local hunters found the burnt wreckage and the skeletonized remains of the bodies on a marshland, some 10 km southwest of Serov Airport.
Probable cause:
It is believed that the airplane was flying at low height when its left lower wing impacted tree tops. The aircraft then rolled to the left to an angle of 90° and crashed, bursting into flames. At the time of the accident, the pilot and other occupants were intoxicated.

The crew was engaged in a spraying mission in the region of Knyazhichi, Sumy Oblast, Ukraine. Following 15 successful sorties that day, the crew was ready for the 16th and started the takeoff procedure from the third of the runway which is 768 metres long. After a course of 577 metres, the single engine aircraft failed to get airborne and overran. The crew decided to abort but this was too late. The aircraft sank in muddy field and rolled for about 56 metres before coming to rest upside down. Both occupants escaped uninjured while the aircraft was damaged beyond repair.
Probable cause:
The following factors were identified:
- The crew did not use the entire runway length for the takeoff procedure (577 metres instead of the 768 metres available),
- The decision of the crew to reject takeoff was taken too late,
- The wind direction changed during the takeoff procedure and became a tailwind component.

The crew departed Şirna Aerodrome around 0830LT on a local spraying mission. About 15 minutes into the flight, the crew encountered engine problems and elected to make an emergency landing. He found a suitable terrain and attempted to land when the aircraft collided with a small concrete wall and some bushes before coming to rest in an open field. Both pilots escaped uninjured and the aircraft was damaged beyond repair.
Probable cause:
Failure of the crew who selected an unsuitable terrain for an emergency landing (rough terrain) and did not take into consideration the wind component.

While flying at a height of about 20 metres, preparing for a new low pass, the single engine aircraft collided with power cables, stalled and crashed upside down in an open field, bursting into flames. The aircraft was destroyed and the pilot was seriously injured.
Probable cause:
The cause of the accident without casualties to the plane An-2 RA-81514 was a violation of the established safe obstacle clearance in the performance of aviation-chemical works, which led to a collision with the lightning protection wire transmission line and the ground. A contributing factor was the performance of an incomplete flight crew. According to the An-2 Flight Manual the minimum crew for aerial chemical works is the commander of the aircraft and a copilot.

The single engine aircraft was dispatched in Baranikovskiy to perform a crop spraying mission on rice paddy field. Before flight, the tank was refueled with 250 litres of E95 fuel. The technician told the pilot the fuel was abnormally yellow but the pilot decided to proceed with the flight. Prior to take off, he asked the copilot to stay on ground and replaced him by the owner of the zone to be treated which is against the published procedures. During the takeoff roll, the aircraft did not accelerate as expected but the pilot continued. After liftoff, at a height of about 30 metres, the pilot initiated a 90° left turn when the engine lost power. He started to drop the load of chemicals then attempted an emergency landing when the aircraft impacted ground. It continued for about 134 metres then struck an irrigation drain, nosed over and came to rest, bursting into flames. The pilot was killed and the passenger was seriously injured.
Probable cause:
The accident was caused by a loss of engine power because the aircraft has been refueled with fuel dedicated to automobile.
The following contributing factors were identified:
- The pilot's lack of knowledge about the flight area and the layout of cultivated fields,
- The pilot failed to brake properly during the emergency landing,
- The absence of a copilot on board,
- The Operator certificat was revoked 3 days prior to the accident.
