Circumstances:
The airplane departed Moscow-Khodynka Airport on a local test flight, carrying one passenger and two crew members, one pilot and one mechanic. After takeoff, while in initial climb, the left engine failed. From a height of about 90-100 metres, the airplane initiated a left turn, stalled, entered a flat spin and crashed in the Petrovski Park located near the Air Force Academy not far from the airport, bursting into flames. The pilot was seriously injured and the flight mechanic and the sole passenger were killed.
Probable cause:
For undetermined reasons, the left engine failed shortly after takeoff, at a critical stage of the flight, causing the airplane to lose height quickly and making it impossible to recover. The pilot, although experienced on the PS-84, had no experience on this type of aircraft and even less experience in engine failure conditions.

Following a hard landing, the left main gear collapsed. The twin engine aircraft named 'Zambeze' veered off runway and came to rest, bursting into flames. There were no casualties but the airplane was partially destroyed by fire.

En route to Tobruk, the airplane suffered an engine failure. The crew attempted an emergency landing in a desert area located some 16 km northwest of El Salloum. Upon landing, the brakes failed and the left main gear collapsed. The aircraft came to rest and was damaged beyond repair. All occupants escaped uninjured.

On final approach to Armstrong at night, the twin engine aircraft hit tree tops and crashed in a wooded area located one mile short of runway, bursting into flames. The aircraft was totally destroyed and all 12 occupants were killed.
Probable cause:
The accident occurred in predawn darkness and adverse meteorological conditions, with an overcast at around 1,000 feet and a visibility of approximately 1.5 mile in light snowfall, but its cause could not be determined despite exhaustive independent inquiries by both the airline and Canadian government authorities.

The aircraft was completing a cross continent flight from Germiston, South Africa, to Heliopolis, Egypt, with intermediate stops in Dodoma, Tanzania, and Nairobi, Kenya. The crew attempted to take off in poor weather conditions (strong winds and heavy rain falls). Shortly after take off from Nairobi-Wilson Airport, while climbing to a height of some 500 feet, the twin engine aircraft rolled to the left, entered an uncontrolled descent and crashed in a huge explosion. All six occupants were killed.

The twin engine aircraft named 'Loch Invar' departed Stockholm-Bromma Airport at 0042LT bound for Heston via Perth, Scotland. After the stop in Perth, the aircraft deviated from the prescribed course by 10 miles to the west. At night, the airplane impacted the slope of a mountain located near Loch Lomond. All five occupants were killed.
