Circumstances:
While flying over a mountainous area of the State of Hidalgo in poor weather conditions, both left and right engines failed almost simultaneously. The captain, Juan Tilghman Gallo, also GM of the company, elected to reach the Actopan Airfield and reduced his altitude from 14,000 to 9,500 feet but was unable to do it. He eventually decided to perform an emergency landing in a field located in El Arenal, south of Actopan. While the aircraft was damaged beyond repair, all occupants escaped uninjured.
Probable cause:
It was determined that the double engine failure was caused by a broken fuel line.

Circumstances:
Shortly after lift off, while in initial climb, the right wing failed. The aircraft went out of control and crashed in a huge explosion. All 16 occupants were killed.
Probable cause:
It appears the loss of the right wing was caused by a structural failure due to excessive vibrations accumulated during operations on difficult and rough terrain.

Shortly after takeoff from Wau Airport, the right engine failed. The airplane banked right, lost height and crashed in a river located near the airfield. The pilot was injured and the aircraft was damaged beyond repair. The airplane was performing a cargo flight, carrying a load of wood.

Loss of control after takeoff due to the failure of the right engine.

Few minutes after takeoff from Tarapacá Airport, bound for Tres Esquinas, the three engine airplane caught fire and crashed in a dense wooded area located few km from the airport. SAR operations were quickly initiated but the burnt wreckage was found two days and a half later. All 16 occupants were killed.
Probable cause:
For unknown reasons, the airplane suffered a sudden and severe in-flight fire, maybe due to the explosion of an engine.
