Crash of an Antonov AN-8RU in Kiev: 7 killed

Circumstances:
The crew (pilots, technicians and engineers) were involved in a local test flight at Kiev-Gostomel Airport. The goal of the mission was to test the JATO propulsion system in case of an engine failure on the Antonov AN-8. For this test program, the aircraft was renamed AN-8RU version. Twenty seconds after takeoff, while climbing to a speed of 220 km/h, the JATO system was ignited. Six seconds later, once the speed of 264 km/h was reached, the left engine was voluntarily shut down. At this time, the crew was unable to feather the propeller that went into auto-rotation. The airplane banked left to a 70-80° angle then dove into the ground and crashed in a field located 1,850 meters past the runway end. The aircraft was totally destroyed and all seven crew members were killed.
Probable cause:
The exact cause of the accident could not be determined with certainty. However, the crew encountered difficulties, was unable to and/or forgot to feather the propeller on the left engine. This caused an asymmetric thrust and high drag which caused the aircraft to be uncontrollable and to crash.

Circumstances:
On a crop-spraying flight from a very small airstrip at Svetilnoye (Borispol district of the Kiev region) when came in too high for landing. Crew failed to go around and forced the aircraft down. It lost speed, came down hard 9 metres before the landing T, nosed over and caught fire. The copilot was killed while both other occupants (among them a mechanic) were injured.
Probable cause:
Wrong approach configuration on part of the captain who adopted a too low approach speed that caused the aircraft to stall. At the time of the accident, the crew was tired due to an insufficient rest time and an excessive consumption of alcohol the night before the crash.

Circumstances:
While flying over the region of Kiev, the crew contacted ATC about a fire on engine number 4 and requested the permission to divert to Borispol Airport. Due to the position of the aircraft, ATC vectored the crew to Zhulyani Airport. Shortly later, while on descent, the engine number four detached and the fire spread to the right wing. Out of control, the airplane entered a dive and crashed in flames in a near vertical attitude in a wooded area located in Tarasovichi, about 41 km north of Zhulyani Airport. The airplane disintegrated on impact and all 34 occupants were killed, among them 4 Yemen, 5 Algerians, one Ugandan and one Afghan. The five Algerians were Mustapha Ferroukhi, ambassador of FLN in Peking travelling with his wife and three children. The Ugandan citizen was John Muhima Kalekezi, president of the Uganda National Congress who was travelling to Moscow for a meeting with Nikita Khrushchev.
Probable cause:
A leak on a fuel injector caused the fuel to flow in the engine nacelle and to ignite while in contact with high temperature equipment. The fire spread to the combustion chamber, causing the engine to catch fire, to fail and eventually to detach.

Circumstances:
The crew was performing a local training flight at Kiev-Zhuliany. While flying at an altitude estimated between 250 to 300 meters, the aircraft collided with an Aeroflot Ilyushin II-14G registered CCCP-L2071 that was approaching the same airport following a cargo flight SU126 from Sofia with five crew members on board. Investigations determined that the propeller of the right engine of L2071 struck the right wing of the L1360, proving that both airplanes were facing each other. Following the collision, L1360 lost its right wing, dove into the ground and crashed while L2071 lost its right propeller that penetrated the cockpit and killed instantly both pilots. Out of control, the airplane also dove into the ground. Both aircraft crashed onto several houses located 4,5 km from the airport. All 9 crew members were killed as well as six people on the ground. 23 others were injured, 12 of them seriously.
Probable cause:
It was determined that the collision was the consequence of a lack of radar coverage, guidance and assistance on part of the ATC at Kiev-Zhuliany Airport. It was reported that the crew of L2071 was not informed about the presence of a second aircraft performing training maneuvers in the vicinity of the airfield at the same altitude.

The crew consisting of two pilots and three instructors left Kiev-Zhuliany Airport at 0936LT on a training mission to Byshev, Borodyanka and back to Zhuliany. While cruising west of Kiev at an altitude of 1,000 meters in good weather conditions, the aircraft went out of control, lost several pieces and went into a dive. At a height of 50 meters, three crew members bailed out and the aircraft crashed at an angle of descent of 35 to 40° in a field located in Gorbovichi, about 24 km west of Kiev. The aircraft was destroyed upon impact, several debris were scattered on a large area and all five crew members were killed. Unfortunately, the three crew members who bailed out were also killed as the distance with the ground was insufficient.
Probable cause:
It was determined that the loss of control was due to the failure of the wings' structure and several other parts of the fuselage due to a faulty design of the aircraft and unacceptable manufacturing defaults.

Circumstances:
Following a flight from Dniepropetrovsk to Kyiv, the pilot reported technical problems with various instruments such as the horizon and the speed indicator. Several repairs were made and the pilot decided to perform a local test flight. Four engineers and two employees of the airport boarded the plane. After takeoff, at a height of about 100 metres, the pilot completed a series of five turns when the airplane rolled to the right, descended to the ground and crashed near Brovary. Four passengers were killed and three other occupants were injured.
Probable cause:
The accident was the consequence of the following factors:
- The airplane' speed was estimated to be 170 km/h prior to the series of turns then dropped to 110 km/h during the last turn, causing a stall,
- The pilot failed to pay attention to the instruments and probably lost his bearings.
