The airplane departed Busselton-Margaret River Airport at 1532LT on a fire fighting mission under callsign Bomber 139 (B139). Following a flight at FL290, the crew descended to the Fitzgerald River National Park and completed a circuit over the area. After the retardants were dropped, the crew elected to gain height when the airplane crashed in a wooded area, bursting into flames. Both pilots evacuated with minor injuries and the airplane was destroyed by fire.

After liftoff from Nasir Airfield, the single engine airplane encountered difficulties to gain height. It rolled to the right, impacted terrain and crashed in an open field, bursting into flames. All occupants evacuated but an elderly passenger later died from injuries sustained. The airplane was totally destroyed by a post crash fire.

The pilot departed Tampa Executive Airport at 1550LT on a cargo flight to Birmingham, Alabama. About two hours and a half into the flight, while approaching Birmingham-Shuttlesworth Airport from the southeast at an altitude of 5,800 feet, the pilot initiated a 180 turn in an apparent attempt to divert to Sylacauga Airport. Shortly later, the twin engine airplane impacted the ground and crashed into trees some 5 km north of Sylacauga Airport which is located 60 km southeast of Birmingham-Shuttlesworth Airport. The pilot was transported to local hospital but seems to be uninjured.

Circumstances:
The single engine airplane departed Ust-Kara on a schedule service to Naryan-Mar with an intermediate stop in Karatayka, carrying 10 passengers and two crews. Approaching Karatayka, the crew encountered icing conditions and decided to divert to Varandey. Engine power was at maximum but the airplane was unable to climb above 200 metres. In such conditions, the captain decided to attempt an emergency landing when the airplane crashed landed in a snow covered area located some 10 km northwest of Karatayka. A passenger and the captain were killed while 10 other occupants were injured. Shortly before the accident, the crew reported severe icing conditions.
Probable cause:
An emergency landing was necessary because of the inability to continue the flight due to the impact of icing on the aircraft's aerodynamic and thrust characteristics.
The following contributing factors were identified:
- The captain's failure to consider potential risks of the aircraft encountering icing conditions when making the decision to take off;
- The captain's delayed decision to change the flight plan despite receiving information about deteriorating meteorological conditions at the destination, which fell below the established thresholds for VFR (Visual Flight Rules) night flights and indicated the possibility of aircraft icing;
- Non-compliance with VFR rules regarding prolonged flight in actual icing conditions.

The single engine airplane landed last October at Dade-Collier Airport, in the center of the Everglades National Park, following a flight from Sancti Spíritus, Cuba. The pilot defected Cuba and landed safely in the US. On November 14, in unclear circumstances, maybe while being transferred to Opa Locka Airport, the airplane crashed in a marshy area located 25 km west of Opa Locka Airport. Both occupants were uninjured and the aircraft was damaged beyond repair.

En route, weather conditions deteriorated with low clouds and heavy rain falls. As the airplane failed to arrive at destination, SAR operations were initiated and the wreckage was found two days later in a dense wooded area located in South Kivu, about 139 km southwest of Goma. The airplane was destroyed by a post impact fire and all three occupants were killed.

On the afternoon of 3 October 2022, a Pilatus Britten-Norman Islander BN2A-21, registered VHWQA and operated by Torres Strait Air, was conducting a non-scheduled passenger air transport flight from Saibai Island Airport, Queensland (QLD) to Horn Island Airport, QLD. There was 1 pilot and 6 passengers (students) on board. About 19 km NE of Moa Island both engines began to surge. The pilot diverted towards Kubin Airport on Moa Island. As the aircraft passed to the south of the township of Saint Pauls, the pilot determined there was insufficient altitude remaining to reach the airport. As a result, the pilot conducted a forced landing on a road 7 km ENE of Kubin Airport. There were no reported injuries to the pilot or the passengers. The aircraft was substantially damaged.
Probable cause:
The ATSB found that the dual engine speed fluctuations and associated power loss was probably the result of fuel starvation. The mechanism was not conclusively determined, however it was identified that the pilot did not operate the aircraft's fuel system in accordance with the aircraft flight manual, and that the configuration and location of the aircraft’s fuel controls and tank quantity gauges were probably not conducive to rapid and accurate interpretation. The aircraft manufacturer released a service letter in June 2022 that detailed an optional modification to centralize the fuel system controls and gauges, however this modification was not fitted to VH-WQA. The ATSB considered that these factors increased the risk of inadvertent fuel tank selection.
