The approach to Takoradi was completed in low visibility due to poor weather conditions. On final, the aircraft hit a hut with its undercarriage and few seconds later, on touchdown, the undercarriage collapsed. The aircraft slid on its belly for several yards before coming to rest. There were no casualties but the aircraft was written off.
Probable cause:
Too low approach.

The crew was completing a night training sortie. On final approach in poor weather conditions, the aircraft hit a hill and crashed few km short of runway. The aircraft was destroyed by impact forces and all three crew members were killed.
Probable cause:
According to the RAF, the minimum descent altitude was fixed at 1,200 feet on approach to the airfield and the crew was able to descend down to 700 feet only when the runway lights were in sight. For unknown reason, the crew did not respect this procedure and continued the approach at an unsafe altitude, well below the prescribed 1,200 feet.

Few minutes after takeoff from Swartkop AFB, in the suburb of Pretoria, the crew noticed fuel streaming from the port fuel tank. The aircraft caught fire, forcing the crew to attempt an emergency landing in an open field. The aircraft came to rest in flames and while all four crew members escaped uninjured, the aircraft was partially destroyed by fire (center section, port engine, port fuselage and port elevator).

Probable cause:
The camouflage paint on the wings of the Dakota did not easily reveal the position of the fuel filler caps and the fact that the port filler cap was not locked tightly in place was not noticed during the preflight. After takeoff from Swartkop Airbase, the hot exhaust gases ignited the fumes and the airplane caught fire in flight, requiring an emergency landing.
