Circumstances:
Upon landing at night on runway 04 at Nyala Airport, the airplane was shot down by Sudan Armed Forces. The airplane veered off runway to the left and crashed in a ditch located 900 metres past the runway threshold, bursting into flames. The airplane was totally destroyed and all 19 occupants were killed, among them 17 members of the Rapid Support Forces (RSF) including Abdul Rahim Wad Abuk, uncle of Hemedti, commander of the RSF. Sudan Government reported the airplane was shot down because suspected of carrying weapons and army equipment on behalf of the rebels. It is believed that the airplane was still operated with the registration N726CK while registered to a Kenyan Operator with 7Q registration (Malawi).
Probable cause:
Shot down by the Sudan Armed Forces.

The airplane departed Venezuela on a flight to Honduras, carrying an unknown number of people. According to local Authorities, the flight was illegal and the crew did not receive any authorization to enter the Honduran airspace. The airplane was probably engaged in an illegal flight and is believed to have crashed while the crew attempted to land in shallow water of a swampy area located in the Warunta River. The airplane was destroyed and nobody was found on site. Honduras State Police also reported that no drugs was found in the airplane. If still operated under registration N10TN, registered to Pacific RIM Management LLC.

Apparently engaged in an illegal mission, the twin engine airplane crashed in unknown circumstances in a plantation located in the region of Chaparral, State of Portuguesa, Venezuela. The airplane was destroyed by a post crash fire and both occupants were killed. The registration may be false.

Circumstances:
The pilot departed Stockton Airport on a solo flight to Chino. On a standard approach in rainy conditions, the airplane suffered two altitude deviations which the pilot attributed to a problem with the autopilot. Towards the end of the flight, the airplane descended past the Minimum Vectoring Altitude (MVA) of 7,400 feet and was issued an altitude alert by the controller, but there was no response from the pilot. The airplane entered an uncontrolled descent with a rate of about 10'000 feet per minute until it crashed in mountainous terrain. The airplane was destroyed by impact forces and the pilot was killed.

The pilot reported that while taxiing, after using a self-serve fuel station, the airplane’s left wing-tip fuel tank struck a post at the fuel station and a fire ignited. The pilot shut down the airplane’s engines and disembarked the airplane with his passenger. The left wing and fuselage were substantially damaged. The pilot reported that there were no preaccident mechanical malfunctions or failures that would have precluded normal operation.
Probable cause:
The pilot’s failure to maintain clearance from the fuel station, resulting in a ground collision and fire.

The pilot reported that he was under the impression that his airplane’s inboard fuel tanks had been topped and he had 202 gallons on board prior to departure. He had a “standing order” with the airport’s fixed base operator to top the tanks; however, the fueling was not accomplished and he did not visually check the fuel level prior to departure. He entered 202 gallons in cockpit fuel computer and unknowingly commenced the flight with 61 gallons on board. Prior to reaching his destination, his fuel supply was exhausted, both engines lost all power, and he performed a forced landing in a cemetery about one mile from the airport. The pilot and his passenger had minor injuries. Inspectors with the Federal Aviation Administration examined the wreckage and determined that damage to the wings and fuselage was substantial. The pilot reported that there were no preaccident mechanical malfunctions or failures with the airplane that would have precluded normal operation.
Probable cause:
The pilot’s improper preflight inspection of the airplane’s fuel system, resulting in him commencing the flight with an inadequate fuel supply.
