The crew was completing a local test flight at Rheine Airfield when the airplane crashed for unknown reasons in the vicinity of the airport. All three crew members were killed.

The airplane departed RAF Marham at 1850LT on an operation to Emden. Approaching the target area, it was shot down by enemy fire. The crew indicated on w/t that an engine was on fire then the airplane crashed into the sea off the Frisian Islands. All six crew members were killed.

The airplane departed RAF Leeming at 1843LT on September 20 on an operation to Berlin. It is believed that it was shot down by enemy fire while approaching the target area. All five crew members were killed.

The airplane departed Münster-Handorf Airfield on an operation to England. It suffered an engine failure and crashed in Metelen. Three crew members were killed and a fourth was injured.

The airplane departed RAF Wyton in the evening of September 15 on an operation to Hamburg. It was shot down by a Me.110 and crashed in Hemslingen, killing the entire crew.
