Circumstances:
In the early afternoon, the twin engine airplane departed Toowoomba Airport on an aerial survey flight under callsign BDOG370 to Mount Isa with three people on board. The purpose of the flight was to conduct a survey mission of the area in case of bushfire. While cruising at an altitude of 28,000 feet, the airplane entered an uncontrolled descent. It reached a rate of descent of 9,600 feet per minute until it crashed in an uninhabited area located approximately 70 km southeast of Cloncurry. All three occupants were killed.

On 25 June 2004, the Australian Transport Safety Bureau released its final investigation report into an accident which occurred on 27 November 2001 at Toowoomba aerodrome, Qld, involving a Beech Aircraft Corporation King Air C90 aircraft, registered VH-LQH, which experienced an engine failure shortly after takeoff. The aircraft was destroyed and all four occupants sustained fatal injuries.
Probable cause:
In light of a further review of the evidence, the ATSB has reconsidered its original finding that the initiating event of the engine failure of VH-LQH was a blade release in the compressor turbine and proposes that an alternative possibility could have been that the initiating event occurred in the power turbine. Notwithstanding this possibility, in either scenario, the remainder of the findings and safety recommendations contained in the original ATSB report are still relevant.

While cruising at an altitude of 27,000 feet in good weather conditions and just passing over the city of Adavale, the twin engine aircraft went out of control and entered a dive. It descended for about 7,000 feet when excessive g loads caused both wings and the fuselage to breakup. The wreckage was found about 3,5 km south of Adavale and all 12 occupants were killed.
Probable cause:
There is insufficient evidence to determine the circumstances and factors which led to the inflight breakup of the aircraft.
